股骨颈骨折固定方式研究

         

摘要

To get the best selection in three aspects of the fixation angle of single screw,the number of screws and the combination modes of screws for the best stability of internal fixation treatment of femoral neck,this paper built a more accurate three-dimensional finite element model.The model mentioned above is based on Lindon mode and also includes different influencing factors.The accuracy of this model exhibits in nonlinear material quality based on the relationship between the property parameters of bone material and the gray-scale value of CT images,thus the simulation of physiological loading condition also increases the accuracy.Through the results of model group including different fixation angles of single screw,each optimized fixation angle of single screw corresponding to different angles of fracture surface is obtained.The analysis of model group of the number of screw shows that the number of screw is the more the better when the surgical conditions permit.In addition,the conclusion of model group of the combination of screws indicates the following features: Inverted triangle placement is little better than triangle patterning in case of three screws,three screws of triangle placement is superior to the selection of two screws,but the decision of the placement of two screws including flat form and diagonal form depends on the angle of fracture surface.%为了对股骨颈骨折内固定治疗方法中的螺钉固定角度、螺钉数量和螺钉组合方式进行合理选择以达到最佳的复位稳定性,建立基于Lindon分型方式,包含不同影响因素,更精确的股骨颈骨折内固定的三维有限元模型,并依据材料属性参数与CT图片灰度值之间的关系为模型赋予非非线性的材料性质,模拟生理载荷条件对模型施加载荷.通过对螺钉固定角度模型组的分析,得到不同骨折面角度所对应的最佳的单钉固定角度;通过对螺钉数量模型组的分析,可知在手术条件允许的情况下螺钉数量越多越好;通过对螺钉组合方式模型组的分析,结果表明:3钉呈倒三角放置略优于3钉呈正三角放置,3钉呈正三角放置优于2钉,而2钉平放与2钉斜放的优劣取决于骨折面角度.
