等离子射频消融术治疗腺样体肥大的术中创伤程度及术后免疫功能评估

         

摘要

Objective: To study the intraoperative wound degree awell apostoperative immune function and ventilation function of plasmradiofrequency ablation fothe treatmenof adenoid hypertrophy.Methods: total of 118 children with adenoid hypertrophy who underwensurgical treatmenin ouhospital between May 2013 and April 2016 were selected and divided into the control group (n=59) who accepted conventional endoscopicutting aspiratoand the observation group (n=59) who accepted the plasmradiofrequency ablation according to the single-blind randomized controlled method.Before and afteoperation, the levelof peripheral blood trauma-related indexes, humoral immunity and complemenindexeawell anasal ventilation function indexewere determined.Results: Before operation, the differencein trauma-related indexes, humoral immunity and complemenindexeawell anasal ventilation function indexewere nostatistically significanbetween two groupof children (P>0.05).Immediately afteoperation, the peripheral blood SP, PGE2 and CRP contentof observation group were lowethan those of control group (P<0.05);24 h afteoperation, peripheral blood IgG, IgM, IgA, C3 and C4 contentof observation group were lowethan those of control group (P<0.05);1 week afteoperation, TIand TElevelof observation group were lowethan those of control group while NPV and MCSlevelwere highethan those of control group (P<0.05).Conclusions: Plasmradiofrequency ablation fotreatmenof adenoid hypertrophy causesmalleintraoperative wound and hamore advantagein optimizing the body'immune function and ventilation function.%目的:研究等离子射频消融术治疗腺样体肥大的术中创伤程度及术后免疫功能、通气功能.方法:选择在本院接受手术治疗的腺样体肥大患儿118例,按照单盲随机对照法分为接受常规鼻内镜下切割吸引刀的对照组59例,接受等离子射频消融术的观察组59例.手术前后,测定外周血创伤相关指标、体液免疫及补体指标、鼻通气功能指标水平.结果:术前,两组创伤相关指标、体液免疫及补体指标、鼻通气功能指标的差异无统计学意义(P>0.05).手术结束即刻,观察组外周血SP、PGE2、CRP含量低于对照组(P<0.05);术后24 h,观察组外周血IgG、IgM、IgA、C3、C4含量低于对照组(P<0.05);术后1周,观察组TIR、TER水平低于对照组,NPV、MCSA水平高于对照组(P<0.05).结论:等离子射频消融术用于腺样体肥大治疗,术中创伤较小,在优化机体免疫功能及通气功能方面更具优势.
