载脂蛋白E基因多态性对阿托伐他汀调脂作用的影响

         

摘要

Objective:Detecting ApoE gene polymorphisms by gene chip technology, and study the relationship between ApoE polymorphisms and the response of atorvastatin. Methods: Patients with hypercholesterolemia were recruited in cardiovascular department of the Fifth People′s Hospital of Dongguan from July 2016 to October 2016. The gene chip technology was used to detect the ApoE gene polymorphisms. All patients with hypercholesterolemia were treated with 20mg atorvastatin every night for 12 weeks. The levels of serum TC,TG,LDL-C,and HDL-C were measured. We compared the difference of the lipid response among different ApoE genotypes. Results:The proportion of ApoE ε3 allele was 70. 9%. It is respectively the most common genotype. The baselines of TC and LDL-c in the group carryingε3 alleles are higher than the group non-carryingε3 alleles, but not statistically significant. After treatment, There was a significant difference in the changed value of TC,LDL-C,HDL-C between the groups before and after treatment. The changes of TG were not statistically significant. Conclusion:1. In this study, the ε3 allele is the most frequently allele;2. There was a correlation between the ApoE gene polymorphisms and the effect of atorvastatin on lipid response. Patients carrying ε2 allele was more prominent efficacy than non-carrying ε2 allele.%目的:运用基因芯片检测高脂血症患者载脂蛋白E(ApoE)基因型,研究ApoE基因多态性对阿托伐他汀调脂作用的影响.方法:选取2016年7月至2016年10月期间,在东莞市第五人民医院心内科住院的高脂血症患者,通过基因芯片方法检测研究对象的ApoE基因型,口服阿托伐他汀治疗12周,每晚20 mg,比较治疗前后血脂谱变化.结果:入选的高脂血症患者共151例,ApoE等位基因 ε3比例为82.1%,为最常见的等位基因型.血脂水平治疗前各等位基因组间均差异无统计学意义.治疗后分析:①TC治疗后均值对比,ε2组与ε3组、ε3组与ε4组对比差异有统计学意义(P<0.001).ε2组与ε4组间对比未见显著差异.提示ε3组治疗后均值水平高于其他2组.②LDL-C治疗后3组均值比较,对比ε2组与ε3组、ε3组与ε4组存在差异;ε2组与ε4组对比未见差异.提示ε3组治疗后LDL-C均值水平同样高于ε2组和ε4组.③HDL-C治疗后均值比较,组间存在差异(P<0.05);ε2组与ε3组、ε2组与ε4组对比差异有统计学意义;ε3组与ε4组对比未见显著差异.提示治疗后ε2组HDL-C水平高于其余2组.④TG治疗后组间无显著性差异(P>0.05).结论:本研究观察人群中,ε3是ApoE基因多态性中最常见的等位基因;ApoE基因多态性与阿托伐他汀调脂效果存在相关性,阿托伐他汀对携带ε2等位基因的人群的TC、LDL-C、HDL-C作用强于非携带人群.
