外翻膝骨性结构的形态学及分型研究

             

摘要

Objective This study evaluates the morphology of the valgus knee with three dimen-rnsional (3D) knee models reconstructed by the MIMICS software , and recommends the new classification ofrnvalgus knee. Methods In order to investigate the main source of valgus deformity in patients , the CTrnvolume data from patients with valgus knee are collected and imported into MIMICS software to recon -rnstruct the 3D model of the valgus knee. Results Osseous defects or dysplasia on the distal aspects of lat-rneral femoral condyle are the most common deformity in valgus knees . Nonetheless , the main source ofrndeformities can come from metaphysic of femur, lateral femoral condyle or metaphysic of tibia. Conclu-rnsion We suggest a new classification based on the morphological characters in the valgus knee . Thisrnclassification helps surgeons define deformed section , and predict the tension of lateral collateral ligament .rn Consequently, adequate lateral soft-tissue release can be achieved and implants with minimal constraintrncan be used to carry out maximum knee stability .%目的 精确测量国人外翻膝骨性结构的三维形态,归纳外翻膝畸形的新分型.方法 收集CT扫描的外翻膝容积性数据,导入MIMICS软件,重建外翻膝三维模型,研究外翻畸形的主要来源.结果 (1)股骨外侧髁的畸形是外翻膝的最常见畸形;但通常并非外翻膝的最严重畸形.(2)股骨外侧髁、股骨干骺端、胫骨干骺端都可能成为外翻畸形的主要来源.结论 按照外翻膝的形态学特点提出了外翻膝的新分型,可在术前判断外侧副韧带的张力情况,有助于术中选择合适的软组织松解顺序,便于采用最适宜的术式矫正畸形.
