完全电视胸腔镜与小切口开胸肺癌根治术对患者手术前后情绪的不同影响

摘要

目的：对比完全电视胸腔镜与小切口开胸肺癌根治术前后患者焦虑、抑郁水平。方法选取2012年1月-2013年6月行肺癌根治术患者109例，根据术式分为完全电视胸腔镜组（C-VATS）、胸腔镜辅助小切口开胸组（VAMT）以及完全电视胸腔镜中转小切口开胸组（下文简称中转组），运用焦虑自评量表（SAS）、抑郁自评量表（SDS）评估患者术前1d、术后3d及1月的焦虑、抑郁情况，并对相关数据进行统计学分析。结果SAS测评结果显示，术前1d、术后3dC-VATS组与VAMT组差别有统计学意义（P＜0．05），术后1月2组差别无统计学意义（P＞0．05）；VAMT与中转组术前1d差别有统计学意义（P＜0．05），术后3d、1月差别均无统计学意义（P＞0．05）；C-VATS组与VAMT组术后3d差别有统计学意义（P＜0．05），术前1d、术后1月差别无统计学意义（P＞0．05）。SDS测评结果显示，3组术前、术后差别均无统计学意义（P＞0．05）。结论对行肺癌根治术患者，C-VATS在围手术期对患者情绪的影响显著低于VAMT。%Objective To compare the status of anxiety and depression in patients with NSCLC af-ter completed video-assisted thoracotomy(C-VATS) versus video-assisted mini-thoracotomy(VAMT) . Methods From January 2012 to June 2013 ,patients with lung cancer treated in our hospital by operation combined with C-VATS ,VAMT and C-VATS to VAMT (Hereinafter called Conversion group) were as-sessed respectively by the self-rating anxiety scale (SAS) and the self-rating depression scale (SDS) one day before operation ,three days after operation ,and one month after operation . Then the obtained data were analyzed statistically . Results The anxiety scores was different significantly between C-VATS group and VAMT group one day before operation and three days after operation (P<0 .05) . The anxiety scores between C-VATS group and VAMT group one month after operation had no significant difference (P>0 .05) . The anxiety scores was different significantly between VAMT group and Conversion group one day before operation (P<0 .05) . There is no significant difference of anxiety scores between VAM T group and transit group three days after operation ,and one month after operation . The anxiety scores was different significantly between C-VATS group and Conversion group three days after operation (P<0 .05) . There is no significant difference of anxiety scores between C-VATS group and transit group one day before operation and one month after operation ,both and the depression scores between the 3 groups (P>0 .05) . Conclusions For the patients with NSCLC surrounding surgery ,the impaction of C-VATS significantly lower than VAMT .
