中药联合刺络拔罐治疗带状疱疹急性期的临床观察

摘要

目的 观察中药联合刺络拔罐在治疗带状疱疹急性期的临床疗效并探讨其机制.方法 选取带状疱疹急性发作的患者85例,按就诊先后顺序随机分为治疗组43例和对照组42例,所有患者采用西医常规治疗,治疗组加用自拟活血解毒汤口服并湿敷患处,并在前3d予梅花针循疱疹经络走行叩刺后拔罐放血.两组疗程均为7d,观察治疗后止疱时间、皮损愈合时间、治疗后7d、10d、1个月疼痛指数和1个月后的后遗神经痛发生率,以及治疗后1个月的临床疗效.结果 治疗组疱疹停止增多时间、皮损愈合时间与对照组比较均明显缩短(均P＜ 0.05).两组治疗前疼痛指数差别不大(P＞0.05).两组治疗后与治疗前比较疼痛指数降低,疼痛明显减轻(均P＜ 0.05),且治疗组同期均优于对照组(均P＜0.05).治疗组总有效率95.35％,高于对照组的80.95％(P＜0.05).治疗组治疗1个月后后遗神经痛所占百分比2.33％,低于对照组的16.67％(P＜0.05).结论 中药联合刺络拔罐治疗带状疱疹疗效显著,具有较强的中医美容学意义.%Objective:To observe and evaluate the clinical efficacy of Chinese herbal medicine combined with bloodletting puncture and cupping in the treatment of acute stage of herpes zoster and explore its mechanism.Methods:85 patients with acute attack of herpes zoster were selected and randomly divided into the treatment group and the control group according to their order of treatment.Among them,43 cases were in the treatment group and 42 cases in the control group.All patients received routine treatment.The treatment group was treated with Huoxue Jiedu Decoction orally and wet dressing,and in the first 3 days,plum blossom needle followed herpes meridian to bleed and cup.In both groups,7～14 d was a treatment course.The following data were observed:the time of ceasing new blister,the healing time of the skin,the pain index on 7 d,10 d and 1 month after treatment,and the incidence of sequela neuralgia after 1 month,and the clinical effect of 1 month after treatment.Results:In the treatment group,the duration of herpes cessation increased,and the healing time of the skin lesions was significantly shorter than that of the control group (both P＜0.05).There was no difference in pain index between the two groups before treatment(P＞0.05).The pain index of the two groups was lower than that before treatment,and the pain was obviously alleviated (both P＜ 0.05),and the treatment group was better than that of the control group (P＜ 0.05).The total effective rate of the treatment group was 95.35％,which was higher than that of the control group (80.95％)(P＜ 0.05).1 month after treatment,the percentage of sequela neuralgia in the treatment group was 2.33％,which was lower than that of the control group(16.67％) (P＜ 0.05).Conclusion:The combination of Chinese herbal medicine and bloodletting puncture and cupping treatment on herpes zoster is effective,and has a strong significance in Chinese medicine cosmetology.
