护肾除癜方辅助西医药物治疗过敏性紫癜肾炎疗效及对凝血指标的影响

摘要

Objective:To observe the effect of Hushen Chudian Decoction assisting Western medicine on henoch schonlein purpura nephritis and the influence on Coagulation indexes.Methods:60 patients were divided into the control group and the observation group(30 cases) by random digital table,treated with Western medicine only and Hushen Chudian Decoction,respectively.The following data was compared:the recent curative effect,clinical symptom scores before and after treatment,urinary red blood cell count,24 h urinary albumin,fibrinogen (FIB),two D-dimer (D-D),fibrin degradation products (FDP),whole blood viscosity,whole blood low shear viscosity,high shear reduced viscosity,low shear whole blood viscosity reduction,platelet aggregation rate and plasma viscosity.Results:The total effective rate of short-term treatment in the observation group was significantly high-er than that of the control group(P＜0.05).After treatment,the skin purpura,lack of energy and fatigue,hand foot heat,limb edema,heavy body weight and dry mouth and less fluid scores in the observation group were significantly lower than those in the control group and before treatment (P＜0.05).The urine red cell count and 24 h urine albumin in the observation group were significantly lower than those in the control group and before the treatment (P＜0.05).The levels of FIB,FDP and D-D in the observation group were significantly lower than those of the control group and before the treatment (P＜0.05).Whole blood viscosity,whole blood low shear viscosity,high shear reduced viscosity,low shear whole blood viscosity reduction,platelet aggregation rate and plasma viscosity were significantly lower than those of the control group and before treatment (P＜ 0.05).Conclusion:Hushen Chudian Decoction assisting Western medicine on allergic purpura nephritis can significantly reduce symptoms,protect renal function,and improve blood coagulation index,helpful to reduce blood viscosity.%目的 观察护肾除癜方辅助西医药物治疗过敏性紫癜肾炎疗效及对凝血指标的影响,并探讨其机制.方法 选取过敏性紫癜肾炎患者60例,以随机数字表法分为对照组与观察组各30例,分别给予西医药物单用和在此基础上加用护肾除癜方辅助治疗.比较两组患者近期疗效,治疗前后临床症状评分、尿红细胞计数、24 h尿白蛋白量、纤维蛋白原(FIB)、D-二聚体(D-D)、纤维蛋白降解产物(FDP)、全血高切黏度、全血低切黏度、全血高切还原黏度、全血低切还原黏度、血小板聚集率及血浆黏度.结果 观察组患者近期治疗总有效率显著高于对照组(P＜0.05);观察组患者治疗后皮肤紫癜、少气乏力、手足心热、肢体浮肿、身体困重及口干少津积分均显著低于对照组、本组治疗前(P＜0.05);观察组患者治疗后尿红细胞计数和24 h尿白蛋白量均显著低于对照组、本组治疗前(P＜0.05);观察组患者治疗FIB、FDP及D-D水平均显著低于对照组、本组治疗前(P＜0.05);观察组患者治疗后全血高切黏度、全血低切黏度、全血高切还原黏度、全血低切还原黏度、血小板聚集率及血浆黏度均显著低于对照组、本组治疗前(P＜0.05).结论 护肾除癜方辅助西医药物治疗过敏性紫癜肾炎可显著减轻相关症状体征,保护肾脏功能,改善凝血指标,并有助于降低血液黏稠度.
