急性缺血性中风治则治法初探

摘要

目前研究表明缺血性中风急性期治疗是关键,因此有必要深入探讨中医对缺血性中风急性期的治则治法.本文从古代医籍和现代研究中对单一证和兼夹证治法的历史渊源、理论依据、具体方法及临床疗效进行分析探讨.发现古代医家多用单一证治则治法,着重于主要证型的解决,临证时抓住病机之关键;现代医家则认为中风病因病机较为复杂,应采取多元化治疗手段,倾向于运用同时涵盖多种证型的兼夹证治法则.结论分析为活血祛瘀法和化痰通络法可能是缺血性中风治则治法的关键,不同治疗法则之间参考标准的建立有助于最优方案的选择,其解决需要更进一步研究.%Research has so far proved that acute stage is the key to the treatment of ischemic stroke,so it is necessary to further explore the therapeutic principle and methods of acute ischemic stroke with TCM.The paper probes into the historical origin,theoretical basis,specific methods and clinical curative effect of single syndrome treatment and complex syndrome treatment from ancient Chinese medical books and modern researches.Ancient physicians widely used single treatment,focused on the main syndromes,and seized the vital pathogenesis.Modern physicians believe that the etiology and pathogenesis of stroke is more complex and should take diversified means of treatment.They tend to use complex syndrome treatment and principles at the same time.The conclusion is that activating blood circulation to remove stasis and eliminating phlegm and freeing channels methods may be the key to ischemic stroke treatment.The establishment of reference standards between different therapeutic principles contributes to choosing the optimal scheme,which needs further research to solve.
