心理护理在肾肿瘤伴下腔静脉瘤栓患者围手术期的应用

摘要

Objective To explore the effect of psychological nursing on different patients with renal cell carcinoma (RCC)involved with inferior vena cava (IVC)tumor thrombus treated by nephrectomy and thrombectomy. Methods 6 9 patients were divided into control group and observation group.In ob-servation group,systematic psychological nursing with cognitive-behavioral interventions were per-formed.The incidence rates of anxiety in two groups were investigated and the satisfaction rates of treatment in two groups were evaluated.All patients were followed up within 1 2 months after opera-tion to investigate the progression-free survival rate. Results Compared with control group,there were lower incidence rates of anxiety in experimental group,and higher 1 2-month progression free survival rate and better satisfaction rate also occurred in experimental group. Conclusions System-atic psychological nursing care with cognitive-behavioral interventions can improve satisfaction rates of pa-tients,reduce incidence of anxiety symptoms in patients and after the treatment of patients with RCC associated with IVC tumor thrombus.Systematic psychological nursing care could improve the pa-tients'rehabilitation.%目的探讨心理护理在肾肿瘤伴下腔静脉瘤栓患者的围手术期中的作用。方法将患者分为两组，其中对照组行常规的心理护理和宣教，观察组为具有完整设计的经认知行为干预的心理护理组。对两组患者术前和术后焦虑反应发生率进行评估，对患者及家属的满意度进行统计，并根据临床资料及术后随访对两组患者的疗效进行比较。结果两组患者术前临床资料无明显差异；观察组患者术前及术后焦虑发生率较低(P<0.05)，患者及家属满意度高于对照组(P<0.05)；观察组患者术后12个月内疾病无进展率高于对照组(P<0.05)。结论对肾癌伴下腔静脉瘤栓患者围手术期采取积极有效的心理护理干预措施，可改善患者焦虑情绪，提高患者及家属的满意度，促进术后恢复。
