经尿道前列腺电切术后应用硬膜外麻醉自控微泵防治术后膀胱痉挛性疼痛及因疼痛而导致的出血

Objective To evaluate the effects of patient controlled epidural analgesia(PCEA) on pain and bleeding after transurethral resection of prostate (TURP). Methods 89 TURP pa-tients collected from 2009.3 to 2014.6 were divided into group A and group B. Group A was treated with PCEA,and group B was given traditional analgesia. Pain and bleeding after operation were compared. Results The visual analogue scale (VAS)and occurrence rate of bleeding in group A were all less than those in the group B (P < 0.05). Conclusions PCEA can effectively control bleeding and pains of patient after TURP.%目的：观察硬膜外麻醉自控微泵镇痛对经尿道前列腺电切术（TURP）术后膀胱痉挛性疼痛及早期出血等并发症的防治效果。方法将2009年3月至2014年6月行 TURP 的89例患者分为 A、B 两组，A 组术后应用硬膜外麻醉自控微泵，B 组应用传统的镇痛药，肌注盐酸哌替啶或静滴间苯三酚。分别记录两组病例术后膀胱痉挛性疼痛发生情况及早期出血例数，以视觉模拟疼痛评分（VAS）判断疼痛程度，并进行比较。结果术后早期患者膀胱痉挛性疼痛的 VAS 及创面出血的发生率 A 组均小于 B 组（P ＜0.05）。结论TURP 后应用硬膜外麻醉自控微泵可明显减轻术后膀胱痉挛性疼痛，并有可能减少出血等并发症的发生。
