气腹和血管生成抑制剂对结肠癌种植转移的影响

         

摘要

Objective To discover the effect of C02 pneumoperitoneum on colon carcinoma growth and metastasis, and to investigate the influence and significance of C02 pneumoperitoneum and an-giogenesis inhibitor on vascular endothelial growth factor( VEGF )expression in colon carcinoma. Methods The CC531 human colon adenocarcinoma cells were injected into abdominal cavity of 38 nude mice,simulating peritoneal spread of colon cancer. These mice were randomly divided into 3 groups: the anesthesia control group( A,n = 10 );the CO2 pneumoperitoneum group( B,re = 14 )and the CO2 pneumoperitoneum with Endostar chemotherapy group( C,n = 14 ). Two days after laparoscopic operation,group C were administrated with TNP - 470( 2mg intraperitoneal injection +0. 6 mg subcutaneously every 48 h )for 14 days. Four weeks later,mice were sacrificed and their tumor weights were observed, and 38 samples of colon carcinoma tissues were examined for VEGF protein expression by EnVision immunohistochemical method. Results The average tumor weights of group B was more than group A( P = 0. 0168 )and group C( P = 0.0018 ); The protein expression levels of VEGF in the tumor tissues of group B were higher than group A ( P =0. 036 )and group C( P = 0. 021 ). Conclusion CO2 pneumoperitoneum can promote metastasis of colon carcinoma in some phases, and early administration of angiogenesis inhibitor can provide favorable prevention and treatment effects.%目的 观察CO2气腹对结肠癌生长与转移的影响;探讨CO2气腹及血管生成抑制剂对血管内皮生长因子(VEGF)在结肠癌组织中表达的影响及意义.方法 使用人结肠癌细胞(CC531)悬液分别注入38只裸鼠腹腔,模拟结肠癌腹腔播散种植.将实验鼠随机分成3组:麻醉控制组(A组);CO2气腹组(B组);CO2气腹+血管生成抑制剂组(C组).C组结束气腹后第2天开始给予血管生成抑制剂(TNP-470)2mg腹腔注射+0.6mg/48h皮下注射,共14d,4周后处死实验鼠,将肿瘤组织分离称重,检测38只种植瘤VEGF蛋白的表达.结果 B组裸鼠平均瘤体重量较A组(P=0.0168)和C组(P=0.0018)高;B组裸鼠结肠癌组织中VEGF蛋白的表达水平高于A组(P=0.036)和C组(P=0.021).结论 CO2气腹对一定生长状态的结肠癌有促进种植转移的作用,术后早期予以血管生成抑制剂化疗有较好的防治作用.
