[Objective]To explore the effect of continuous positive airway pressure(CPAP) on insulin resist ance(IR) in elderly patients with type 2 diabetes and obstructive sleep apnea hypopnea syndrome (OSAHS).[Methods] Fiftytwo elderly patients with type 2 diabetes and OSAHS were enrolled in this study. All patients re ceived CPAP treatment for 3 months. The change of homeostasis model assessment(HOMA) index reflecting IR was observed before and after treatment. [Results] After 3 months of CPAP treatment, the apnea/hypopnea index (AHI) and miniSpO2 were improved significantly compared with that before treatment( P <0.01). The HOMA index was significantly decreased after 3 months of CPAP treatment compared with that before treatment(3.62±0.86 vs 1.98 ± 0.78, P <0.01). [Conclusion]CPAP treatment can significantly improve the IR in elderly patients with type 2 diabetes and OSAHS.%[目的]探讨持续气道正压通气(CPAP)对老年合并2型糖尿病的阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者胰岛素抵抗(IR)的影响.[方法]选择老年合并2型糖尿病的OSAHS患者52例,通过3个月的CPAP治疗,观察治疗前后反映IR的稳态模式评估(HOMA)指数变化.[结果]CPAP治疗3个月后呼吸暂停/低通气指数(AHI)及最低脉搏血氧饱和度(SpO2)(miniSpO2)较治疗前明显改善(P<0.01), HOMA指数较治疗前明显降低(3.62±0.86 vs 1.98±0.78,P<0.01).[结论]CPAP治疗可显著改善老年合并2型糖尿病的OSAHS患者的IR.
