丹红注射液联合前列地尔治疗老年心功能不全伴不稳定心绞痛疗效观察

摘要

【目的】探讨丹红注射液联合前列地尔治疗老年心功能不全伴不稳定心绞痛患者的临床疗效。【方法】选择2011年9月至2015年9月重庆市急救中心老年医学科收治的180例老年心功能不全伴不稳定心绞痛患者，根据治疗方案的不同将180例患者分为前列地尔组、丹红注射液组和联合治疗组，比较三组患者临床疗效、心电图和血流动力学指标变化及不良反应的发生情况。【结果】三组患者临床疗效和心电图疗效比较，联合组的临床疗效和心电图诊断疗效总有效率高于前列地尔组和丹红注射液组，差异有统计学意义（χ2＝185．52， P ＜00．5；χ2＝154．02， P ＜00．5）。三组患者血流动力学指标比较，与联合治疗组比较，前列地尔组、丹红注射液组的左室射血分数、心排量、心脏指数、左心室工作指数均小于联合组，差异有统计学意义（均 P ＜00．5）。三组患者低血压、脑出血、皮下出血、心率失常的发生率比较，差异均无统计学意义（均 P ＞00．5）。【结论】丹红注射液联合前列地尔治疗心功能不全伴不稳定心绞痛的老年患者，临床疗效较好，不良反应发生率较小，有一定的临床应用价值。%[Objective]To explore the clinical curative effect of Dan red injection combined with Alprostadil in the treatment of elderly cardiac insufficiency with unstable angina pectoris .[Methods]One hundred and eighty ca‐ses of elderly cardiac insufficiency with unstable angina pectoris admitted by the Chongqing Emergency Center from Sept 2.011 to Sept 2.015 were selected .According to different methods of treatment ,patients were divided into the Alprostadil group ,the Dan red injection group and the combined group .Clinical curative effect ,electrocardio‐gram curative effect ,hemodynamic index and the incidence of adverse reactions of the three groups were compared .[Results]Comparison of clinical curative effect of three groups showed that the total effective rate and the electro‐cardiogram curative effect of the combined group was higher than those in the Alprostadil group and the Dan red injection group ,the difference was statistically significant (χ2 =18 5.52 ,P <0 0.5) .Hemodynamic‐index compari‐son of the three groups revealed that the left ventricular ejection fraction ,cardiac output ,cardiac index ,left ven‐tricular work index of the Alprostadil group and the Dan red injection group were less than those of the combined group ,the difference was statistically significant ( P <0 0.5) .In the incidence comparison of low blood pressure , cerebral hemorrhage ,subcutaneous hemorrhage and arrhythmia of the three groups ,the difference was not statis‐tically significant (mean P >0 0.5)[.Conclusion] Dan red injection combined with alprostadil in the treatment of elderly cardiac insufficiency with unstable angina pectoris gets a sound clinical curative effect with low adverse re‐action rate .The method has some clinical application value .
