目的 探讨慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者使用BiPAP呼吸机通气的疗效.方法 将42例患者随机分为两组,在相同的常规治疗下,治疗组22例加用BiPAP呼吸机通气治疗,对照组20例加用呼吸兴奋剂治疗,观察治疗前后两组血气分析结果的pH值、PaCO2及PaO2的变化.结果 治疗组与对照组治疗后pH值,PO2,PCO2比较均有显著性差异(P＜0.05),疗效明显优于对照组.结论 BiPAP呼吸机无创正压通气治疗能显著降低COPD合并Ⅱ型呼吸衰竭患者的二氧化碳分压,改善缺氧,疗效确切.%Objective To evaluate the clinical effect of noninvasive bilevel positive airway pressure (BiPAP) ventilator on patients of COPD with type II respiratory failure. Methods 42 patients who were identified COPD with Type II respiratory failure were divided into two groups randomly. Under the same conventional treatment, 22 cases in the treatment group were additionally treated with BiPAP ventilator, 20 cases in the control group were additionally treated with respiratory stimulant. The changes of blood gas analysis( pH, PCO2 and PO2) before and after the treatment were observed and compared in two groups. Results Significant difference in curative effect existed between the treatment group and the control group (P < 0. 05). The treatment group significantly outweighed the control group. Conclusions BiPAP ventilator can significantly lower PaCO2, improve PaO2 in the patients of chronic obstructive pulmonary disease with Type II respiratory failure.
