目的：比较现行常规的结核诊断方法的优劣并寻找最经济有效的诊断方法组合。方法分析我院确诊为结核病(包括肺结核和肺外结核)76例与非结核病39例,对以上病例进行比较 PPD、结核抗体、T-SOPT 这三种诊断方法的敏感性、特异性并得出结果。同时比较临床方面花费及性价比。结果115例患者中,协助诊断结核病的三种检测方法的灵敏度分别为 T-SOPT 92．11%,PPD 为63．16%,结核抗体为50%；特异性分别为 T-SOPT 89．74%,PPD 为92．31%,结核抗体为58．97%；出结果时间分别是48 h,48~72 h,24 h。结论相比传统结核检测方法,T-SOPT 在结核病检测中有着更高的灵敏度和可信性,对结核病有更强的诊断价值。%Objective To compare the advantages and disadvantages of current routine TB diagnosis meth-ods, in order to find the most economical and effective diagnosis combination. Methods 76 patients with tuberculo-sis and 39 cases of non-tuberculosis were selected. Their sensitivity and specificity of different diagnosis results were compared, including PPD, tuberculosis antibody and T-SPOT. Meanwhile, the clinical cost and performance were compared. Results Among the 115 cases, the sensitivity of T-SPOT was 92. 11% , 63. 16% of PPD, and 50% of tuberculosis antibody. The specificity of T-SPOT was 89. 74% , 92. 31% of PPD, and 58. 97% of tuberculosis anti-body. The time required for the results of T-SPOT was 48 hours, 72 hours for PPD, and 24 hours for tuberculosis an-tibody. Conclusion Compared to conventional test methods for tuberculosis, T-SPOT shows a higher sensitivity and specificity in the clinical diagnosis of tuberculosis.
