目的：分析常频呼吸机联合大剂量氨溴索治疗新生儿呼吸窘迫综合征( RDS)的疗效。方法选择我院2011年3月-2014年3月72例RDS患儿,采用隐匿数字随机法分为两组,均采用常频呼吸机并给予综合治疗,对照组36例患儿给予肺泡表面活性物质,观察组36例患儿给予大剂量氨溴索静脉滴注,比较两组疗效、血气指标、临床相关指标及肺炎发生情况。结果治疗后48 h两组 PH 、PaO2、PaO2/FiO2较治疗前明显升高,PaCO2较治疗前明显下降(P<0．05),两组治疗前后各指标比较(P>0．05)。两组平均吸氧时间、机械通气平均时间、平均住院时间比较差异无显著性,观察组总有效率为86．11%,对照组总有效率为83．33(P>0．05)。观察组平均住院费用明显低于对照组,并发肺炎患儿明显少于对照组(P<0．05)。结论常频呼吸机联合大剂量氨溴索治疗RDS疗效与PS联合呼吸机基本相同,但费用较低,可减轻患儿家庭负担,及降低肺炎的发生率。%Objective To analyze of the curative effect of ventilator combined with large dose of ambroxol in the treatment of respiratory distress syndrome of newborn. Methods 72 infants with RDS were randomly divided into two groups. All infants were given constant frequency respirator and comprehensive treatment. 36 cases in the control group were given pulmonary surfactant, and 36 cases in the observation group were given large dose of ambroxol hydrochloride. The incidence of pneumonia, curative effect, and blood gas indexes were compared between the two groups. Results After the treatment, the levels of PH, PaO2 , and PaO2/FiO2 increased significantly, and the level of PaCO2 decreased significantly (P<0. 05). There was no significant difference in average oxygen time, mechanical ventilation time, and average duration of hospital between the two groups ( P>0. 05 ) . The total effective rate was 86. 11% in the observation group, and 83. 33% in the control group (P>0. 05). The average hospitalization expen-ses in the observation group was significantly lower than that in the control group. The incidence of pneumonia was significantly lower in the observation group than in the control group (P<0. 05). Conclusion The curative effect of normal frequency ventilation combined with large dose of ambroxol hydrochloride is similar with PS combined with ventilator in the treatment of RDS, but its cost is lower, and it can reduce the incidence of pneumonia.
