PCT、Hs-CRP和AIS-APS对SAP风险评估的价值

         

摘要

Objective To explore the value of procalcitonin ( PCT) , high-sensitive C-reactive protein ( Hs-CRP) and acute ischemic stroke-associated pneumonia score ( AIS-APS) to evaluate the risk of stroke-associated pneumonia (PCT). Methods From June 2014 to March 2016, 60 patients with acute stroke were consecutively en-rolled from the neonatal intensive unit ( NICU) in our hospital. The patients were divided into the SAP group and the control group. The serum PCT and Hs-CRP were immediately tested and AIS-APS was assessed when they were in hospital. The patients who had SAP were divided into low-risk (0-13 points), intermediate-risk (14-20 points) and high-risk (21-35 points), according to AIS-APS. Results There were 24 patients with SAP (40. 0%). The levels of PCT, Hs-CRP and AIS-APS in the SAP group were significantly higher than those in the control group ( t=25. 92, P<0. 05;t=2. 05, P<0. 05;t=12. 66, P<0. 05). There was significantly correlation with the incidence of SAP among low-risk, intermediate-risk and high-risk (χ2=20. 77, P< 0. 05). Conclusion PCT, Hs-CRP and AIS-APS have important value on evaluating the risk of stroke-associated pneumonia, and AIS-APS provides evi-dences for different treatment.%目的 探讨降钙素原(PCT)、超敏C反应蛋白(Hs-CRP)及急性缺血性卒中相关性肺炎评分(AIS-APS)对缺血性卒中相关性肺炎(SAP)风险评估的价值.方法 选择2014年6月-2016年3月在我院神经重症监护室住院患者共60例,根据是否患SAP分为SAP组和对照组,入院时测定每位患者血清PCT、Hs-CRP及进行AIS-APS量表检查.根据AIS-APS分为低危组(0-13分)、中危组(14-20分)、高危组(21-35分).结果 共24例患者发生SAP(40.0%),与对照组相比,SAP组PCT、Hs-CRP、AIS-APS均升高(tP=25.92 P<0.05;th=2.05P<0.05;ta=12.66 P<0.05).低危组、中危组、高危组SAP发生率差异具有统计学意义(χ2=20.77 P<0.05).结论 PCT、Hs-CRP、AIS-APS对SAP风险评估具有重要价值,AIS-APS为SAP提供了分层诊治的依据.
