目的 探讨12例布氏杆菌性脊柱炎误诊为脊柱结核的原因,提高对两种疾病的认识,降低误诊率.方法 回顾本院12例误诊病例资料,并进行临床、实验室检查及影像学表现的综合分析.结果 12例误诊病例均因对布氏杆菌性脊柱炎影像表现认识不足,没有全面分析临床、病史、实验室检查及流行病学而误诊.经详细询问病史,并结合流行病学、实验室及影像学检查而纠正.结论 临床医师在疑难病症不能确诊时,在结合实验室与影像科客观数据分析的同时,应密切结合流行病学、临床症状进行综合分析,降低疾病误诊率.%Objective T o explore the causes of 12 cases of brucellosis spondylitis misdiagnosed as spinal tuberculosis ,improve the understanding of tw o diseases and reduce the misdiagnosis rate .Methods T he data of 12 misdiagnosed cases in our hospital w ere review ed ,and clinical laboratory examinations and ima‐ging findings w ere analyzed comprehensively .Results All of the 12 misdiagnosis cases w ere caused by the lack of knowledge of the imaging manifestations of brucellosis and the lack of complete analysis of the clin‐ical ,medical history ,laboratory tests and epidemiology .T hey w ere eventually corrected by more detailed medical history and more comprehensive laboratory examination and pathological examination .Conclusion When the clinicians cannot make a definite diagnosis ,they should com bine objective data from laborato‐ry and imaging department , meanw hile ,closely co mbine epidemiology ,clinical sy mptoms to conduct a comprehensive analysis ,to reduce the rate of misdiagnosis of diseases .
