Objective To evaluate the clinical results of treatment for the posterolateral tibial plateau fractures via the modified posterolateral approach and lateral plate. Methods Seventeen cases of posterolateral tibial plateau fractures were treated by a modified posterolateral approach and lateral plate. Results All patients were followed up for 12~22 months. Bone union was obtained in all cases in 8 ~16 weeks. There were no injury of the common peroneal nerve, no infection of incisional wound and skin necrosis, no loss of reduction and breakage of internal fixation, no varus or valgus of the knee. According to Rasmussen knee joint function scores criterion,ther results showed 10 cases were excellent,5 cases good and 2 fair. Conclusions The modified posterolateral approach and lateral plate can fa-cilitate the reduction and fixation for posterolateral tibial plateau fractures, which has many advantages such as good exposure,less invasion and the excellent clinical result.%目的：探讨改良后外侧入路外侧钢板固定治疗胫骨平台后外侧骨折的临床疗效。方法对17例胫骨平台后外侧骨折患者行改良后外侧入路外侧钢板固定治疗。结果患者均获得随访，时间12~22个月。骨折均愈合，时间8~16周。无腓总神经损伤，无切口感染或皮肤坏死，无骨折复位丢失及内固定断裂，无膝关节内外翻畸形。根据Rasmussen膝关节功能评分进行评定：优10例，良5例，可2例。结论改良后外侧入路外侧钢板固定治疗胫骨平台后外侧骨折具有显露清楚、内固定放置安全、创伤小等优点，临床疗效满意。
