目的 通过64排多层螺旋CT进行梗阻性结肠癌评估,研究其在手术方案选择中的应用价值.方法 将100例梗阻性结肠癌术前患者随机分为2组:观察组术前常规进行纤维结肠镜和活检评估,并进行64排多层螺旋CT评估；对照组术前仅进行纤维结肠镜和活检评估.分析手术选择与评估临床资料的关系.结果 观察组术前各病理分期准确度均显著高于对照组(P＜0.05).观察组手术方案预测符合率显著高于对照组(P＜0.05).2组选择根治术的患者血清C反应蛋白(CRP)和淀粉样蛋白A(SAA)含量均显著低于选择姑息手术的患者(P＜0.01).结论 64排多层螺旋CT评估梗阻性结肠癌患者的策略可显著提高术前分期和手术选择的准确性,降低术后风险,具有较大的临床应用价值.%Objective To study the application value in the operation scheme selection through the assessment of 64 multi-slice spiral CT for obstructive colorectal cancer. Methods A total of 100 patients with obstructive colorectal cancer were randomly divided into two groups. The observation group received the evaluation of 64 multi-slice spiral CT pre-operation, and the control group received colonoscopy and biopsy assessment pre-operation. The relationship between the selection and assessment of clinical data were analyzed. Results The preoperative accuracy rate in the observation group were obviously higher than that in the control group (P < 0. 05). The operation program prediction coincidence rate in the observation group was obviously higher than that in the control group (P<0. 05). CRP and SAA had important influences on the selection of operation (P <0.01). Conclusion The strategies of 64 multi-slice spiral CT in evaluation of patients with obstructive colon cancer can significantly improve the preoperative staging and operation choice accuracy, and reduce the postoperative risk. Thus it has great clinical application value.
