血糖异常及电解质紊乱与新生儿窒息的关系

         

摘要

Objective To explore the neonatal asphyxia glucose and electrolyte disturbances,and the relationship between abnormal blood sugar and electrolyte disorder and the degree of asphyxia.Methods Two hundred and sixty-seven cases of neonatal asphyxia were chosen as research objects.According to the apart score they were divided into mild asphyxia group (n =189)and severe asphyxia group (n =78),and 69 cases of normal newborns were taken as control group.Abnormal blood glucose and electrolyte disturbances were compared between each two groups.Results Among the three groups,the differences in Na+,Cl-,Ca2+ between either two groups showed statistical significance (P < 0.05).Na+,Cl-,Ca2 + and Apgar score of the asphyxia group were positively correlated.The correlation coefficients were as follows:Na+ (r =0.634,P < 0.05),C1-(r =0.586,P < 0.05) and Ca2 + (r =0.727,P < 0.05).As the degree of asphyxia deepened,Na+ Cl-,serum Ca2 + concentration decreased; There was significant difference between each two groups (P<0.05).Mean blood glucose blood glucose was higher in severe asphyxia,the mild asphyxia group of low blood sugar,constituent ratio differences were statistically significant in three groups of hyperglycemia and hypoglycemia (P < 0.05).There were more high blood sugar cases in severe asphyxia group,and there were more low blood sugar cases in mild asphyxia group.Conclusion Asphyxia of newborns often comes along with Na+,Cl-,Ca2+ ions and glucose asphyxia disorder.Close attention should be paid to changes in children with changes of situation and active symptomatic treatment should be given so as to reduce the mortality rate of neonatal asphyxia.%目的 分析窒息新生儿血糖异常及电解质紊乱的情况,探讨血糖异常及电解质紊乱与窒息程度的关系.方法 以267例窒息新生儿为研究对象,按Apgar评分分为轻度窒息组(189例)、重度窒息组(78例),并以69例正常新生儿为对照组,比较各组血糖异常及电解质紊乱情况.结果 3组Na+、Cl-、Ca2+任2组比较差异均有统计学意义(P<0.05),窒息组Na+、Cl-、Ca2+与Apgar评分均呈正相关,相关系数分别为Na+(r-0.634,P<0.05)、Cl-(r =0.586,P<0.05)、Ca2+(r =0.727,P<0.05),随窒息程度加深,Na+、Cl-、Ca2+血清浓度降低;3组血糖均值比较差异有统计学意义(P<0.05),重度窒息组血糖较高,轻度窒息组血糖较低,3组高血糖、低血糖构成比比较均差异有统计学意义(P<0.05),重度窒息组高血糖居多,轻度窒息组低血糖居多.结论 新生儿窒息常伴Na+、Cl-、Ca2+离子的降低及血糖的紊乱,临床需密切关注患儿病情变化,积极对症治疗,以降低新生儿窒息的致残致死率.
