社区无缝链式服务管理模式对中老年慢性高血压患者血压指标、并发症的影响分析

摘要

Objective To study effect of community seamless chain service management on blood pressure indicators and complications in middle-aged and elderly patients with chronic hypertension.Methods A total of 226 middle-aged and elderly patients with chronic hypertension enrolled in this community service center were divided into control group and study group according to the random number table method,with 113 cases in each group.The control group was treated with conventional hypertension management,and the study group was given community-based seamless chain service management intervention model on the basis of control group.The changes of blood pressure level,quality of life and the incidence of complications were compared between the two groups before and after management.Results After 6 months of management,the diastolic blood pressure and systolic blood pressure of the two groups were lower than management before,and study group was much lower (P ＜ 0.05).The quality of life of the two groups was better than management before,and the study group was better than the control group (P ＜ 0.05).The overall incidence of complications in the study group was significantly lower than that in the control group (7.1％ vs.16.8％,P ＜0.05).Conclusion Community seamless chain service management model in middle-aged patients with chronic hypertension has significant effect,and it can effectively control the patient's blood pressure level stability,reduce the incidence of complications,improve the quality of life of patients,so it is worth promoting.%目的 分析社区无缝链式服务管理模式对中老年慢性高血压患者血压指标、并发症控制的影响.方法 选取226例中老年慢性高血压患者,根据随机数表法分为对照组、研究组各113例.对照组患者接受常规高血压管理,研究组患者在对照组基础上实施社区无缝链式服务管理模式护理.比较2组患者管理前后血压水平变化、生活质量及并发症发生率.结果 管理6个月后,2组患者舒张压、收缩压水平均低于管理前,且研究组显著低于对照组(P＜0.05);管理6个月后2组患者生活质量均优于管理前,且研究组显著优于对照组(P＜0.05);研究组患者并发症总发生率7.1％,显著低于对照组的16.8％(P＜0.05).结论 社区缝链式服务管理模式能有效控制患者血压水平稳定,减少并发症发生,改善患者生活质量.
