联合检测凋亡抑制蛋白及脑和急性白血病胞质蛋白在老年急性髓系白血病中的意义

摘要

目的 探讨联合检测凋亡抑制蛋白(Survivin)及脑和急性白血病胞质(BAALC)蛋白表达对老年急性髓系白血病(AML)病情评估、疗效及预后预测的意义.方法 应用Western blot法检测88例初诊老年AML患者骨髓Survivin及BAALC蛋白表达,分析其与老年AML患者疗效和预后的关系.结果 治疗无效患者BAALC与Survivin蛋白的表达量明显高于治疗有效患者,差异均有统计学意义(x2=4.502,P=0.021;x2=4.301,P=0.044).BAALC与Survivin蛋白表达之间呈正相关(r =0.681,P=0.034).生存分析结果显示,Survivin及BAALC蛋白高表达者无进展生存(PFS)率及总生存(OS)率均明显低于Smvivin、BAALC蛋白低表达者,差异有统计学意义(x2 =3.412,P =0.032;x2 =3.052,P =0.039;x2 =3.114,P =0.006;x2 =3.331,P =0.003).结论 在老年AML患者中,Survivin、BAALC蛋白高表达者治疗效果与预后明显差于低表达者.联合检测Survivin及BAALC蛋白表达,有助于老年AML患者的疗效评估及预后估测.%Objective To investigate the clinical and prognostic significance of detection of Survivin combined brain and acute leukemia cytoplasmic (BAALC) protein expression in elderly acute myeloid leukemia(AML) patients.Methods Western blotting was carried out to detect the Survivin and BAALC protein expression levels in 88 elderly AML patients and to analyze the relationship between Survivin and BAALC protein expression levels with clinical outcome and prognosis.Results Compared to relieved patients,both Survivin and BAALC protein in unrelieved patients were statistically higher (x2 =4.502,P =0.021;x2 =4.301,P =0.044).There was a positive correlation between the protein expression of Survivin and BAALC (r =0.681,P =0.034).And patients with high expression of Survivin and BAALC had worse PFS and OS than those with low expression of Survivin and BAALC (x2 =3.412,p=0.032;x2=3.052,P=0.039;x2=3.114,P=0.006;x2 =3.331,P=0.003).Conclusion High expression of Survivin and BAALC may be associated with worse clinical outcomes in elderly AML patients.Combined detection of Survivin and BAALC expression contribute to the evaluation of curative effect and prognosis.
