青年乳腺癌VEGF-C和VEGFR-3的表达及其与临床病理关系的研究

         

摘要

目的 探讨青年乳腺癌患者血管内皮生长因子C(VEGF-C)和血管内皮生长因子受体3(VEGFR-3)表达情况以及与临床特点和病理学特征的关系.方法 收集44例临床病理资料完整的青年乳腺癌病例(年龄在35岁以下),用免疫组织化学方法检测癌组织中VEGF-C及VEGFR-3的表达情况,与患者TNM分期、腋淋巴结转移状况进行比较分析.选取同期45岁以上中老年妇女乳腺癌患者44例进行对照.结果 青年组乳腺癌VEGF-C及VEGFR-3的阳性表达率分别为68.18%及61.36%,中老年组分别为38.64%及40.91%.青年组VEGF-C阳性表达率及程度显著高于中老年组,差异有统计学意义(P<0.01);VEGFR-3阳性表达率及程度也显著高于后者,差异有统计学意义(P<0.05).青年组中VEGF-C与VEGFR-3表达呈正相关(P<0.05),VEGF-C、VEGFR-3与TNM分期、腋淋巴结转移数目呈正相关,差异有统计学意义(P<0.05),随着TNM分期增加及腋淋巴结转移数目的 增多,二者阳性表达率显著升高.结论 青年乳腺癌侵袭性强、预后差可能与癌组织中VEGF-C及VEGFR-3的高表达有关;监测青年乳腺癌组织中VEGF-C及VEGFR-3的表达情况对判断其预后及指导治疗有一定的参考意义.%Objective To study the expression of VEGFC and VEGFR3 in young patients with breast cancer, and the correlation between their expression with clinicopathological characteristics. Methods The expression of VEGFC and VEGFR3 in 44 young patients with breast cancer ( under 35 years old ) during recent years was detected by immunohistochemical method and the clinical data of clinicopathological characteristics such as TNM (tumor, node, metastasis) staging, axillary lymph node metastasis and biological characteristics were retrospectively analyzed and compared. The other 44 patients ( over 45 years old ) with breast cancer in the same period were served as control group. Results Positive rates of expression of VEGFC and VEGFR3 were 68.18% and 61. 36% respectively in these young patients, and 38.63% and 40.91% were found in older patients. The positive rate and degree of VEGFC and VEGFR3 expression in young patients group were significantly higher than those of patients in control group ( P =0. 002,0. 021 ). The expression of VEGFC and VEGFR3 was positively correlated with TNM staging and axillary lymph node metastasis in young patients group ( P < 0.05 ), and increase in positive rate was followed with the progress of TNM staging and axillary lymph node metastasis; and the expression of VEGFC was positively correlated with the expression of VEGFR3. Conclusion Breast cancer in young women with enhanced invasiveness and poor prognosis may be closely related to over - expression of VEGFC and VEGFR3. Monitoring the expression of VEGFC and VEGFR3 is helpful in improving the prognosis and efficacy of therapy in young patients with breast cancer.
