Objective To investigate the clinical curative effect on ovarian chocolate cyst combined with infertility treated with laparosco_py combined with gonadotropic releasing hormone( GnRH-a). Methods 84 patients with ovarian chocolate cyst combined with infertility were randomly divided into two groups,42 cases were in each group. The observation group were treated with laparoscopic GnRH-a. The control group were treated with laparoscopy. The levels of serum estradiol(E2),follicular estrogen(FSH)and luteinizing hormone(LH)of patients in two groups before and after treatment were compared. The VAS scores of the main symptoms of dysmenorrhea,pelvic pain and dyspareunia before and after treatment also were compared. All patients of two groups were followed up for 1 year. The rate of pregnancy and recurrence were compared be_tween the two groups. Results The levels of serum E2,FSH,LH of patients in two groups significantly decreased after treatment( P ﹤0. 05). There was no statistical significance of serum FSH,LH levels between the two groups( P ﹥0. 05). The serum level of E2 of the observation group was lower than that of the control group( P ﹤0. 05). The pelvic pain,dysmenorrhea,dyspareunia VAS scores of patients in two groups after treatment were significantly decreased( P ﹤0. 05). The observation group was lower than that of the control group( P ﹤0. 05). Compared the pregnancy rate of the patients of two groups atⅠ~Ⅱstages,there was no significant difference( P ﹥0. 05). The pregnancy rate atⅢ ~ Ⅳsta_ges of the patients in the observation group was higher than that of the control group( P ﹤0. 05). The total pregnancy rate of the two groups was no significant difference( P ﹥0. 05). Conclusion Laparoscopy combined with GnRH-a is an effective and safe in the treatment of ovarian chocolate cyst. This method can improve pregnancy rate of the patients atⅢ~Ⅳ stages after surgery. And this method can be used as the pre_ferred therapy method in the treatment of ovarian chocolate cyst.%目的：探讨腹腔镜联合促性腺激素释放激素激动剂（ GnRH-a）治疗卵巢巧克力囊肿合并不孕的临床疗效。方法将84例卵巢巧克力囊肿合并不孕患者随机分为两组，每组42例，观察组采用腹腔镜联合GnRH-a治疗，对照组采用腹腔镜治疗。比较两组治疗前后血清雌二醇（E2）、卵泡雌激素（FSH）和黄体生成素（LH）水平，并比较两组治疗前后主要症状痛经、盆腔疼痛及性交疼痛的视觉模拟评分法（ VAS）评分，对两组患者随访1年，比较两组妊娠率及复发率。结果治疗后两组血清E2、FSH、LH水平均显著下降（ P ﹤0.05），血清FSH、LH水平组间比较差异无统计学意义（ P ﹥0.05），观察组血清E2水平低于对照组（ P ﹤0.05）；两组治疗后盆腔痛、痛经、性交痛VAS评分均显著下降（ P ﹤0.05），观察组低于对照组（ P ﹤0.05）；两组Ⅰ~Ⅱ期患者妊娠率比较差异无统计学意义（ P ﹥0.05），观察组Ⅲ~Ⅳ期患者妊娠率高于对照组（ P ﹤0.05），两组总妊娠率差异无统计学意义（ P ﹥0.05）。结论腹腔镜联合GnRH-a治疗卵巢巧克力囊肿安全有效，能够提高Ⅲ~Ⅳ期患者术后妊娠率，可作为卵巢巧克力囊肿合并不孕优选的治疗方式。
