目的：观察法舒地尔治疗老年心脏病患者术后急性肾损伤的临床疗效及安全性。方法53例老年心脏术后急性肾损害病人，随机分为研究组26例和对照组27例。两组均予常规药物治疗，研究组在常规治疗同时给予法舒地尔注射液30 mg +生理盐水50 ml 静脉泵入，每12小时1次，连用7 d。观察治疗前和治疗后3、5、7天两组患者尿量，尿 N -乙酰-β- D -氨基葡萄糖苷酶（NAG）、尿γ-谷氨酰转肽酶（γ- GTP）、尿α1-微球蛋白（α1- MG）、血清肌酐（SCr）、血尿素氮（BUN）的变化，并计算两组患者重症监护病房（ICU）停留时间、对照治疗结束后透析治疗率。研究组患者使用法舒地尔注射液期间观察药物不良反应。结果与对照组同时间点相比，研究组治疗后尿量明显增多，尿 NAG、尿γ- GTP、尿α1- MG、血清肌酐、血 BUN 均明显降低（ P 均﹤0.05）。与治疗前比较，研究组尿量于治疗后7 d 显著升高，尿 NAG、尿α1- MG、尿γ- GTP、血 SCr、BUN 显著下降。研究组 ICU 停留时间短于对照组，差异具有显著性（ P ﹤0.05）。研究组患者使用法舒地尔注射液期间无严重不良反应。结论在常规治疗的基础上加用法舒地尔注射液治疗对老年心脏术后急性肾损伤患者可促进肾功能的恢复，同时安全性高，具有十分重要临床意义。%Objective To explore the clinical effects and safety of Fasudil injection on acute kidney injury after cardiac surgical proce-dures. Methods 53 AKI patients after cardiac surgical procedures were randomly divided into the control group and the study group. All patients received routine therapy,while patients in the study group were additionally given Fasudil injection for 7 days. Urine volume,NAG,γ - GTP,α1- MG,SCr and BUN were measured before and after the control treatment. And the ICU stay duration and the percentage of dialysis after the con-trol treatment were calculated. Results After the treatment,urine volume in the study group was more than that in the control group,while urine- NAG,γ - GTP,α1 - MG,SCr and BUN in the study group were lower than those in the control group. Compared with before treatment,urine volume was more than that in the 7 d group,while urine - NAG,γ - GTP,α1 - MG,SCr and BUN in the study group were lower than those in the 7 d group. The ICU stay duration in the study group was less than that in the control group. Conclusion On the basis of routine therapy,Fa-sudil injection may promote the recovery of renal function in AKI - old age patients after cardiac surgical procedures. At the same time,it has high security with very important clinical significance.
