雾化吸入碱性溶液对广泛耐药鲍曼不动杆菌肺炎小鼠药物敏感性的影响

         

摘要

Objective To determine the effect of alkaline solution atomization inhalation on treatment of acinetobacter baumannii infected pneumonia mice based on the previous study in vitro.Methods Pneumonia mice model was established by atomization inhalation of acinetobacter baumannii.The pneumonia mice was treated with cefoperazone/sulbactam and alkaline solution atomization inhalation was applied.The airway liquid pH of mice was determined.The mortality was determined at 8 h, 24 h and 48 h after antibiotic treatment.The bacterial load and white blood cellwere determined in bmnchoalveolarlavage fluid.The inflammatory cytokines in blood was detected.Hematoxylin and eosin was used to determine the pathological change of lung tissue.Results Compared with the antibiotic treatment group, alkaline solution atomization inhalation significantly increased airway liquid pH (P<0.05), but did not affect the basic characteristics.The atomization inhalation group showed 10% mortality rate less than the antibiotic treatment group at 8 h and 48 h.In addition, the alkaline solution atomization inhalation significantly reduced the bacterial load, white blood cell countin bmnchoalveolarlavage fluid and inflammatory cytokines in blood, but had no effect on pathological changes of the lungs.Conclusion Alkaline solution atomization inhalation significantly improved the effect of cefoperazone/sulbactam for the treatment of pneumonia in mice infected with acinetobacter baumannii.The method was simple and easy, therefore, it has clinical application value.%目的 拟在前期体外实验结果的基础上,利用鲍曼不动杆菌感染肺炎小鼠模型,观察雾化吸入碱性溶液对头抗生素治疗鲍曼不动杆菌感染肺炎效果的影响.方法 采用雾化吸入法构建鲍曼不动杆菌肺炎小鼠模型,给予头孢哌酮钠/舒巴坦钠治疗,同时给予碱性溶液雾化吸入,测定小鼠气道表明液体pH,分别在抗生素处理后的8 h,24 h以及48 h时统计小鼠死亡率,检测支气管肺泡灌洗液细菌负荷和白细胞计数、血液炎症因子水平,采用苏木精/伊红染色确定肺组织病理变化.结果 与给予抗生素治疗的肺炎小鼠相比,碱性溶液雾化吸入均显著提高了小鼠的气道液体pH值(P<0.05),然而并不影响小鼠的基本生命体征.与给予抗生素治疗的肺炎小鼠相比,在鲍曼不动杆菌感染小鼠后8 h及48 h,雾化吸入碱性溶液组小鼠死亡率均降低10%.此外,碱性溶液雾化吸入可显著降低支气管肺泡灌洗液细菌负荷、白细胞计数以及血液炎症因子水平,但对肺部病理结构无明显影响.结论 碱性溶液雾化吸入可明显改善头孢哌酮钠/舒巴坦钠治疗小鼠鲍曼不动杆菌感染肺炎的效果,该方法简便易行,无副作用,具有临床推广价值.
