腹腔镜下子宫血管阻断术联合子宫肌瘤剜除术治疗前后对内分泌激素的影响

摘要

目的 探讨腹腔镜下子宫血管阻断术联合子宫肌瘤剜除术治疗子宫肌瘤对内分泌激素的影响.方法 将70例子宫肌瘤患者采用随机分成对照组和观察组,每组35例.对照组患者行腹腔镜下子宫肌瘤剜除术,观察组行腹腔镜下子宫血流阻断术联合子宫肌瘤剜除术.测定两组患者手术前、术后3个月和术后6个月的血清内分泌生殖激素:卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P).对比两组患者的术中出血量、手术时间、术后排气时间和住院天数.结果 观察组术中出血量和手术时间显著少于对照组,术后住院天数显著短于对照组,差异有统计学意义(P＜0.05).两组的术后排气时间差异无统计学意义(P＞0.05).手术前后对照组各激素水平无统计学差异(P＞0.05).术后3个月观察组患者E2水平较术前显著下降,FSH水平显著上升,差异有统计学意义(P＜0.05),术后6个月E2和FSH水平与术前相比无统计学差异(P＞0.05).结论 行腹腔镜下子宫血流阻断术联合子宫肌瘤剜除术能有效控制术中出血量,缩短手术时间和住院时间,且其内分泌激素的影响是暂时的.%Objective To study the influence of laparoscopic uterine artery occlusion combined with resection of adenomyosis on the endocrine hormone.Methods 70 cases of uterine fibroids were randomly divided into control group and observation group,35 cases in each group.The control group were given resection of adenomyosis.The observation group were given laparoscopic uterine artery occlusion combined with resection of adenomyosis.Serum endocrine hormones of two groups of the patients were measured before the operation,3 months after operation and 6 months after operation,such as:follicle stimulating hormone (FSH),(LH),estradiol (E2),progesterone (P).The amount of bleeding,operation time,postoperative exhaust time and hospital stay were compared between the two groups.Results The amount of bleeding and operation time in the observation group were significantly less than that in the control group,the mean length of stay was significantly shorter than that in the control group,the difference was statistically significant (P ＜ 0.05).There was no significant difference in the postoperative passing flatus (P ＞ 0.05).After 3 months,the level of E2 in the observation group was significantly lower than that before operation,and the level of FSH was significantly increased,the difference was statistically significant (P ＜ 0.05),and there was no significant difference in the level of FSH and E2 after 6 months (P ＞ 0.05).Conclusion Laparoscopic Uterine artery occlusion combined with resection of adenomyosis can effectively control the amount of bleeding,shorten the operation time and the mean length of stay and the effect on endocrine hormone is temporary.
