艾灸热敏化腧穴治疗膝关节骨性关节炎(肿胀型)的临床研究

摘要

目的:通过观察艾灸热敏化腧穴与关节腔注射透明质酸钠治疗膝关节骨性关节炎(肿胀型)的临床疗效对比研究,探索一种新的有效疗法.方法:采用单盲、随机、对照研究方法,将符合条件的50例患者随机分为试验组24例,给予艾灸热敏化腧穴治疗；对照组26例,给予关节腔注射透明质酸钠治疗.两组均为3星期一疗程,共治疗1个疗程(包括不足1个疗程痊愈者).结果:两组组间分别在治疗1星期、2星期时肿胀值有极其显著性差异(P＜0.01),疼痛值、总积分值有显著性差异(P＜0.05)；3星期肿胀值有显著性差异(P＜0.05),疼痛、总积分值无显著性差异(P＞0.05).两组组间在治疗1星期有效率有显著性差异(P＜0.05),2星期、3星期有效率无显著性差异(P＞0.05).结论:艾灸热敏化腧穴对治疗膝关节骨性关节炎(肿胀型)有很好的疗效,能在早期快速有效的缓解各种症状,有望成为治疗膝关节骨性关节炎(肿胀型)一种全新高效的治疗方法,值得进一步研究和临床推广.%Objective: To observe the clinical therapeutic comparision of moxibustion on heat - sensitive acu-points with injection sodium hyaluronate into joint cavity treat KOA, and to explore a new effective therapy. Methods: Single - blind, randomized controlled trial method was adopted, and 50 eligible patients were randomly divided into experimental group of 24 patients received moxibustion on heat - sensitive acupoints; Control group of 26 patients received injection sodium hyaluronate into joint cavity. Three weeks treatment for a course ,totally one course (including the patients who recovered less than one course). Results:The point of swell had an extremely significant difference in the two groups (P ＜0. 01). The pain and all of seven aspcts had a significant difference ( P ＜ 0. 05 ) after the treatment for one week and two weeks; The point of swell had a significant difference in the two groups ( P ＜ 0. 05 ) , but the pain and all of seven aspcts had no significant difference ( P ＜ 0. 05 ) after the treatment for three weeks; The effective rate of the two groups had a significant difference after the treatment for one week(P ＜0. 05) ,but no significant difference after the treatment for 2 and 3 weeks (P ＞ 0. 05). Conclusion: Moxibustion on heat -sensitive acupoints in the treatment of KOA (swelling type) has a very good curative effect,and it can alleviate various symptoms quickly and efficiently in the early days, therefore, it is expected to become a therapeutic method which is brand new and efficient in the treatment of KOA (swelling type) ,and it is well worth further research and clinical expansion.
