目的：探讨不同针刺疗法镇痛在静息态脑功能磁共振局部一致性的变化及针刺镇痛机制的异同性。方法：选取本院腰椎间盘突出患者30例（男14例、女16例）分为传统体针组16例和平衡针组14例。传统体针组针对大肠俞、天应穴，配穴关元俞穴直刺停留，平衡针组对腰痛穴短暂针刺不停留。治疗结束30 min后通过fMRI扫描，并使用SPM5软件处理得ReHo图像。结果：两组ReHo值相同（ P＜0．03）的脑区，包括双侧小脑半球等12个脑区。平衡针组ReHo值显著上升（ P＜0．03）的脑区主要有右侧下顶叶等5个脑区，显著下降（P＜0．03）的脑区主要有左侧中央后回等5个脑区。结论：传统体针与平衡针两种针刺疗法均通过镇痛中枢调节患者痛觉，本实验对两种疗法选穴和针法的差异造成了二者信息传入机制及镇痛效果的不同。%Objective:To analyze the changes of different analgesic acupuncture treatments for regional homoge-neity of brain activity in resting state by functional magnetic resonance imaging and the similarities and differ-ences between their mechanisms.Methods:Thirty cases with lumbar disc protrusion, including fourteen males and sixteen females, were divided into a traditional acupuncture group ( sixteen cases) and a balance acupunc-ture group ( fourteen cases) .The traditional acupuncture group kept acupuncturing at Dachang Shu, Tianying acupoint and Guan Yuan Shu, while the balance acupuncture group was once acupunctured at Yaotong point. 30 minutes after the acupuncture, fMRI was used to scan the patients’ brains and ReHo graphs were collected through SPM5 software.Results:In twelve encephalic regions including cerebellum and so on, the ReHo in both groups hit the close data( P<0.03) .In five encephalic regions including the right inferior parietal lobe and so on, the ReHo in balance acupuncture group was on the rise(P<0.03).In five encephalic regions including the left postcentral gyrus and so on, the ReHo in balance acupuncture group was declined(P<0.03).Conclu-sion:Both of Traditional acupuncture and balance acupuncture can ease the pain through central analgesia.In this experiment, different acupoints and measures lead to differences of afferent mechanisms and analgesia be-tween two groups.
