艾灸联合桃红饮加味治疗瘀血阻络型类风湿性关节炎的临床疗效研究

         

摘要

Objective:To observe the clinical effect of moxibustion combined with modified Taohong Yin in the treatment of rheumatoid arthritis with the pattern of blood stasis blocking channels. Methods:110 cases of rheumatoid arthritis with the pattern of blood stasis blocking channels were randomly divided into the control group (n = 55) and the observation group (n = 55); the control group was treated with modified Taohong Yin; on which basis, the observation group was also treated with moxibustion. Changes of joint function, such as lasting time of morning stiffness, indexes of joint swelling and joint tenderness, joint function and grip strength were measured; indicators of ESR, CRP, RF, IgA, IgG, IgM, C3, and C4 were observed; as well as adverse reactions were compared between the two groups. Results:The total effective rate of the observation group was 90. 91%, which was significantly higher than 74. 55% of the control group(χ2 =5. 153, P = 0. 023). There was no significant differences between the two groups in terms of lasting time of morning stiffness, indexes of joint swelling and joint tenderness, joint function and grip strength before the treatment in the two groups (P > 0. 05); they were all improved after the treatment in the two groups (P <0. 05), in which the observation group was superior to the control group (P <0. 05). In terms of inflammatory indexes and immunological indexes, there were no significant differences between the two groups before the treatment (P >0. 05); they were significantly improved after the treatment in the observation group(P <0. 05). There was no significant difference in adverse reactions between the two group(P >0. 05). Conclusion:Moxibustion combined with modified Taohong Yin can significantly reduce the inflammatory response, correct the immune level, improve the joint function, and improve the curative effect when treating rheumatoid arthritis.%目的:观察艾灸联合桃红饮加味治疗瘀血阻络型类风湿性关节炎的临床疗效.方法:110例瘀血阻络型类风湿性关节炎患者随机分为中药组和艾灸联合中药组,各55例,中药组患者接受桃红饮加味治疗,艾灸联合中药组患者接受艾灸联合桃红饮加味治疗.对比治疗前后关节功能变化,包括晨僵时间、关节肿胀指数、关节压痛指数、关节功能和握力;血沉(ESR)、C-反应蛋白(CRP)、类风湿性因子(RF)、Ig A、Ig G、Ig M、C3、C4等指标;对比两组不良反应发生情况.结果:艾灸联合中药组总有效率90. 91%显著高于中药组74. 55%,两组比较χ2=5. 153,P=0. 023.治疗前两组晨僵时间、关节触痛指数、关节肿胀指数、关节功能评分和握力无显著性差异(P>0. 05);治疗后两组上述指标均显著改善(P<0. 05),艾灸联合中药组较中药组改善更加明显(P<0. 05).治疗前两组炎症指标和免疫学指标差异无统计学意义(P>0. 05),治疗后艾灸联合中药组炎症指标和免疫学指标改善显著(P<0. 05).两组不良反应无显著性差异(P>0. 05).结论:艾灸联合桃红饮加味治疗类风湿性关节炎,可以显著减轻患者炎症反应,纠正免疫水平,改善关节功能,提高疗效.
