计算机导航下髓芯减压加骨髓基质干细胞移植治疗股骨头坏死的临床研究

摘要

Objective To evaluate the safety and accuracy in treating the avascular necrosis of femoral head (ANFH) with computer navigated core decompression and bone marrow stream cell transportation and to guide the clinical treatment. Method Within the prospective study, 36 patients suffered ANFH (ARCO Ⅰ - Ⅱ ) and treated with computer navigated core decompression and bone marrow stream cell transportation were studied. The operating time, blood loss, x ray exposure, preoperative and 6 week postoperative Harris score and imaging evaluation were recorded and compared with conventional core decompression and bone marrow stream cell transportation. Results There were no obvious difference between the two groups in imaging evaluation, operating time and blood loss ( P > 0. 05 ). There were statistical difference between the two groups in x ray exposure and 6 week postoperative Harris score [ (4. 1 ± 1.8 ) s,(13.6±3.2)s,P <0. 01,and89.4±10. 1,83.1±10. 5, P <0.01]. Conclusion Computer navigated core decompression and bone marrow stream cell transportation have good security and precision in treating early stage ANFH.%目的 评估计算机导航下髓芯减压加骨髓基质干细胞移植方法治疗股骨头坏死的安全性及精确性,并指导临床工作.方法 采用计算机导航下髓芯减压加骨髓基质干细胞移植方法治疗的36例ARCO Ⅰ-Ⅱ期股骨头坏死患者进行回顾性研究,统计手术时间、出血量、放射线照射时间,术后6周Harris评分及影像学评估,结果与常规髓芯减压加骨髓基质干细胞移植进行对比分析.结果 两组患者在术后影像学、手术时间、出血量方面差异无统计学意义(P>0.05);而在放射线照射时间及术后6周Harris评分分别为[(4.1±1.8)s,(13.6±3.2)s,P<0.01]和(89.4±10.1,83.1±10.5,P<0.01),差异有统计学意义(P<0.01).结论 计算机导航下髓芯减压加骨髓基质干细胞移植方法治疗早期股骨头坏死具有良好的安全性及精确性.
