目的：对医院门诊药房药品调配差错类型进行分析，以减少甚至杜绝差错的发生。方法对医院门诊药房《药品调配差错登记本》中2014年1月1日～2015年12月31日期间登记的调配差错情况进行汇总分析。结果2年内共发生调配差错120例，其中药品品种差错45例（37.50%），药品数量差错25例（20.83%），药物规格剂型差错16例（13.33%），用法用量差错11例（9.17%），注射单填写差错14例（11.67%），错发患者5例（4.17%），发出药品不合格4例（3.33%）。结论加强针对性管理，规范处方调配程序，制订优质服务规范，提高药学人员业务素质及工作责任心，可有效防范处方调配差错的发生。%Objective To analyze the drμg dispensing error types in the hospital outpatient pharmacy to reduce and even prevent the error occurrence. Methods The dispensing errors recorded in the Drμg Dispensing Errors Registration of the hospital outpatient pharmacy from the 1st January, 2014 to 31th November, 2015were performed the summary and analysis.Results In 2 years,120 cases of dispensing errors occurred,in which,45 cases(37.50%) were the errors of drμg varieties,25 cases(20.83%)were the drμg quantity errors,16 cases(13.33%) were the errors of specifications and formulations, 11 cases(9.17%) were the errors of dosage and usage,14 cases were the errors(11.67%) of filling in the injection list.Conclusion Strengthening the pointed management, standardizing the prescription dispensing procedures,formulating the first-rate quality service standards and improving the professional quality and responsibility of pharmacy staffs can effectively prevent the occurrence of prescription dispensing errors.
