经皮心包穿刺行心外膜射频消融术患者的临床观察和护理

摘要

Objective To investigate the observation and nursing care of patients with epicardial catheter ablation by percutaneous puncture pericardium .Methods The data of 7 patients treated with epicardial catheter ablation by percutaneous puncture pericardium in West China Hospital of SCU between July of 2015 and February of 2016 were analyzed to investigate the clinical observation and nursing care .Results All the Seven patients were treated successfully with epicardial catheter ablation by percutaneous puncture pericardium .One patient suffered from ventricular fibrillation during the operation and was restored to sinus rhythm immediately via external defibrillation .Three patients suffered from bradycardia .No peicardium tamponade occurred during the first 3 days after operation .Five patients complained about chest pain of different degrees .No pericardial effusion was found in Echocardiography and no ST‐T changes were found in Electrocardiogram .There were no arrhythmia recurrence , pericarditis and pericardial effusion in the follow‐ups which lasted for two to three months .Conclusion Full preparations before operation and good cooperation during operation can reduce the complications and guarantee the success of the operation .%目的：探讨心外膜射频消融术患者的临床观察及护理方法。方法分析四川大学华西医院2015年7月至2016年2月7例行心外膜射频消融术患者的临床观察及护理。结果7例患者均经心外膜射频消融治疗成功。1例术中发生室颤，立即行体外电除颤后恢复窦性心律；3例出现心动过缓。所有患者在术后3d内均无心包填塞，5例诉不同程度胸痛，超声心动图检查未发现心包积液，心电图未见S T‐T改变。随访时间2～3月，无心律失常复发，无心包炎和心包积液。结论手术前充分准备，术中良好配合，可减少术后并发症的发生，也是取得手术成功的保障。
