神经外科病人异丙酚血药浓度的检测与药代动力学

             

摘要

In order to investigate the method of monitoring the blood concentration of propofol and the propofol parmacokinetics, 22 neurosurgery patients were infused with propofol 2 mg/kg by induction.The infused time was 30 s.After 1,2,3,4,5,10,15,20 min of induction,the blood concentrations of propofol were monitored with high performance liquid chromatography.The time-dose relationship shape was charted.The high performance liquid chromatography was a good method to monitor the blood concentration of propofol.It took very short time for propofol to get to peak concentration.After 1 min if induction,the blood concentration of propofol was 9.06 mg/L,and decreased rapidly.This result correlated with the fast induction and quick wake of patients.In this study the blood concentration of propofol was higher than in other study after 1 min of induction,perhaps this is related to infused speed and time.%为探讨异丙酚血药质量浓度(以下简称血药浓度)的检测方法及诱导剂量的异丙酚的药代动力学的变化,对22例ASA分级1级的神经外科择期手术病人,在静脉注入异丙酚(2 mg/kg,注药时间30 s)后1、2、3、4、5、10、15、20 min抽静脉血,采用高效液相色谱测定异丙酚血药浓度。结果:从异丙酚和麝香草酚水溶液色谱图、空白血色谱图和病人血样色谱图上可以看出,其中杂质峰、麝香草酚峰和异丙酚峰分离效果好,根据检测结果得到异丙酚血药浓度随时间变化的标准曲线,显示:异丙酚的达峰时间很快,给药1 min血药浓度为9.06 mg/L,之后5 min迅速降低,5~20 min代谢则比较缓慢。
