胃癌根治术后腹腔淋巴漏14例处理体会

摘要

Objective: To investigate the causes and treatments of celiac lymphatic leak of patients underwent radical gastrectomy. Methods: Fourteen cases with lymphatic leak after radical gastrectomy were treated with fasting, total parenteral nutrition, supplement of albumin,supplementary plasma and somatostatin. Results:The incidence rate of celiac lymphatic leak was 1. 99% ,the leak occurrence time was the 4th to 7th day and the maximum drainage volume was 1 800 ml per day after radical gastrectomy. Fourteen patients were all cured. Conclusions: Intraoperative prevention is major measures and conservative treatment is the first choice for lymphatic leak.%目的:探讨胃癌根治术后腹腔淋巴漏的发生原因及治疗方法.方法:对确诊的术后淋巴漏14例患者禁食,完全肠外营养,补充白蛋白,必要时补充血浆,并应用生长抑素治疗.结果:胃癌术后淋巴漏发生率1.99%,发生淋巴漏的时间为术后第4～7天;每天最大引流量为1 800 ml;14例均痊愈.结论:胃癌术后淋巴漏应以术中预防为主;淋巴漏治疗首选支持及保守治疗.
