四腔心切面超声检查在产前诊断先天性心脏畸形中的作用

摘要

目的：探讨四腔心切面超声检查在产前诊断先天性心脏畸形中的临床意义。方法：对2013年7月至2015年9月产前超声诊断为先天性心脏畸形的78例胎儿的资料进行回顾分析。结果：通过四腔心切面超声检查确诊57例，占73．1％，其中27例并发大动脉畸形，占47．4％；提供关键线索13例，占16．7％；四腔心切面正常，但为先天心脏畸形8例，占10．2％。结论：在胎儿先天性心脏畸形产前超声筛查中，四腔心切面检查发挥重要作用，但是需要联合左、右室流出道切面观察，可得到更详细、全面的信息，降低先天性心脏畸形的漏诊率。%Objective:To investigate the clinical significance of four -chamber echocardiography in the diagnosis of congenital heart malforma-tion.Methods:From July 2013 to September 2015, 78 patients with congenital heart malformation diagnosed by prenatal ultrasound in the Second People's Hospital of Zhuhai were retrospectively analyzed.Results:There were 57 cases of congenital heart defects diagnosed in this group, account-ing for 73.1 %.Among them, 27 cases were complicated with aorta malformation, accounting for 47.4 %.13 cases provided the key clue of con-genital heart abnormality, accounting for 16.7%;8 cases were normal in fetal four-chamber view, but presented congenital heart abnormality, ac-counting for 10.2 %.Conclusion:Four-chamber view plays an irreplaceable role in prenatal diagnosis of congenital heart malformation , which can provide much more comprehensive and detailed information if combined with view of the left and right ventricular outflow tract so that the misdi-agnosis rate of congenital cardiac malformations will decline .
