目的 观察中药外敷联合地塞米松甲状腺内注射治疗亚急性甲状腺炎(subacute thyroiditis,SAT)的临床疗效.方法 选取80例SAT患者,随机分为对照组(40例)和治疗组(40例),对照组给予甲状腺局部注射地塞米松磷酸钠注射液治疗,治疗组在对照组治疗基础上加用中药外敷,治疗周期均为4周,治疗前后检测患者红细胞沉降率(erythrocyte sedimentation rate,ESR)和甲状腺功能[血清游离三碘甲状腺原氨酸(free triiodothyronine,FT3);血清游离甲状腺素(free thyroxine,FT4);促甲状腺激素(thyroid stimulatinghormone,TSH)],并进行疗效评价.结果 与治疗前比较,两组患者治疗后FT3、FT4以及ESR水平均减少,TSH含量增多(P＜0.05);治疗组治疗前后FT3、FT4、TSH、ESR差值与对照组比较,差异均有统计学意义(P＜0.05).两组患者临床疗效的分布比较,差异具有统计学意义(P＜0.05),治疗组临床疗效优于对照组.结论 中药外敷联合地塞米松甲状腺内注射可提高SAT的临床疗效,减少复发率.%Objective To investigate the clinical effect of external application of traditional Chinese medicine combined with intrathyroidal injection of dexamethasone in the treatment of subacute thyroiditis (SAT).Methods A total of 80 patients with SAT were enrolled and randomly divided into treatment group and control group,with 40 patients in each group.The patients in the control group were treated with intrathyroidal injection of dexamethasone sodium phosphate injection,and those in the treatment group were given external application of tradition Chinese medicine in addition to the treatment in the control group.The course of treatment was 4 weeks for both groups.Erythrocyte sedimentation rate (ESR)and thyroid function [free triiodothyronine (FT3),free thyroxine (FT4),and thyroid stimulating hormone (TSH)] were measured before and after treatment,and the clinical outcome was evaluated.Results Both groups had significant reductions in FT3,FT4,and ESR and a significant increase in TSH after treatment (P＜0.05),and there were significant differences between the two groups with respect to the changes in FT3,FT4,TSH,and ESR after treatment (P＜0.05).There was a significant difference in the distribution of clinical outcomes between the two groups (P＜0.05).The treatment group had a significantly lower recurrence rate than the control group (P＜0.05).Conclusion External application of traditional Chinese medicine combined with dexamethasone can improve the clinical outcome of SAT and reduce recurrence rate.
