高频超声对男性乳腺发育症的诊断意义

         

摘要

Objective To explore the imaging features of high frequency ultrasonography of gynecomastia and its diagnostic val-ue. Method The sonographic features of the 117 patients with gynecomastia examined by high-frequency Doppler ultrasound were retro-spectively analyzed and conclusion. Results In the 117 patients with gynecomastia,52 cases were pathologically confirmed as gynecomas-tia. 65 cases were confirmed by clinical followed up. The echogenic features of the hyperplastic gland were mainly characterized by hypoe-choic area,whose shape was unequal or patch -like echogenicity,with the nipple in its center or at its lateral side and with a distinct boundary. There was no obvious blood flow signal or dotted blood flow signal on CDFI. All Patients were thickened glands,the thickness of gland layer was 3. 5 mm~20 mm,(8. 48 mm in average). The thickness of gland layer in operative group was(9. 05 ± 3. 85)mm,while it was(7. 38 ± 2. 56)mm non-operative group(P﹤0. 01). Conclusion High frequency ultrasonography which is a simple,accurate,non-invasive characteristics is used as the first choice for inspection method and an effective means for dynamic monitoring. The thickness of gland layer of male mammary gland by ultrasonic measurement is the objective basis for clinical treatment.%目的:探讨男性乳腺发育症的高频超声表现及其诊断意义。方法:回顾性分析行高频超声检查的117例男性乳腺发育症声像图特点并对其进行分析。结果:117例男性乳腺发育症中52例经手术病理证实,65例经临床随访诊断。声像图表现为以乳头为中心呈盘状或偏向一侧的低回声区为主的不均质或斑片状回声,与周围组织分界清晰,其内血流不丰富或可探及点状血流信号。增生腺体厚度3.5~20 mm,平均(8.48±4.37)mm。其中手术组52例,腺体层厚度(9.05±3.85)mm,非手术组65例,腺体层厚度(7.38±2.56)mm,两组比较差异有统计学意义( P﹤0.01)。结论:高频超声具有简便、准确、无创伤等特点,是诊断男性乳腺发育症的首选及动态监测的有效手段。超声测量男性乳腺腺体厚度是临床治疗的客观依据。
