腮腺多形性腺瘤CT诊断及鉴别诊断

摘要

目的：探讨腮腺多形性腺瘤的CT特征。方法：回顾性分析经手术及病理学证实的腮腺多形性腺瘤患者13例，并结合文献资料，分析其CT表现特点，提高诊断准确率。结果：13例腮腺包块均为单侧，12例单发，1例多发。大多数边缘清楚，其中4例可见浅分叶，10例密度较均匀，3例密度不均匀，其中2例有囊变区，1例可见钙化，13例增强扫描中8例呈较均匀轻度强化，5例呈环状或斑片状不均匀轻度强化。结论：根据CT平扫及增强扫描表现特点，并结合多平面重建（ MPR）、最大密度投影（ MIP）重建，可以提高腮腺多形性腺瘤的诊断准确性，最终诊断依靠病理。%Objective To study the CT features of parotid pleomorphic adenoma. Methods Retrospective analysis of the surgery and pa-thology confirmed parotid pleomorphic adenoma in 13 patients,combined with literature date,analysis of characteristics of CT realization,im-prove the accuracy rate of diagnosis. Results 13 cases of parotid gland mass were unilatrral,12 cases with solitary,1 cases of multiple,most clear edge,of which 4 cases were superficial lobulation in 10 cases with homogeneous density,3 cases showed heterogeneous density,there were 2 cases with cystic areas,1 cases with calcification,13 cases with enhanced scan in 8 cases showed homogeneous,5 cases showed circular or patchy inhomogeneous mild enhancement Conclusion caaording to the plain and contrast enhanced CT scan features,combined with multi planar reconstruction(MPR),maximum intensity projection(MIP)reconstruction,can improve the accuracy of diagnosis of parotid pleomor-phic adenoma,but the final diagnosis depends on pathology.
