胸部肿瘤放射治疗中的体位固定技术对摆位误差的影响

         

摘要

Objective To investigate the influence of the hands position on the set-up error in the patients undergoing radiotherapy for thoracic tumor.Methods Clinical data of 93 patients with thoracic tumor received intensity-modulated radiotherapy were retrospectively analyzed.The patients were divided into two groups according to different hands positions during radiotherapy.The patients in group A (54 cases) were positioned with the fingers crossed on dedicated arm support and those in group B (39 cases) the hands were placed on both sides of the body.CT simulation localization was performed for each patient.Tumor target area and organs at risk were delineated on treatment planning system.Cone beam CT scan was performed before the first treatment to measure the set-up error of tumor target area in the two groups.Results The set-up error in left and right direction in group B was less than that in group A (P<0.05).There was no significant difference in the set-up error in up and down direction and front and rear direction between the two groups (P>0.05).The calibration frequency in front and rear direction in group B was less than that in group A (P<0.05). There was no statistically significant difference in the calibration frequency in left and right direction and up and down direction between the two groups (P>0.05).Conclusion To keep two hands on both sides of the body has less influence on the set-up error in the patients undergoing radiotherapy for thoracic tumor.%目的 探讨胸部肿瘤患者接受放射治疗(放疗)时双手的摆放位置对摆位误差的影响.方法 回顾性分析接受调强放疗的93例胸部肿瘤患者的临床资料.根据放疗时患者双手摆放位置的不同分为两组:甲组54例,双手十指交叉置于专用臂托上;乙组39例,双手置于身体两侧.患者均行CT模拟定位,勾画肿瘤靶区和累及器官,制定放疗计划.首次治疗前行机载锥形束CT扫描,对两组患者肿瘤靶区的平移摆位误差进行测量.结果 乙组患者左右方向摆位误差小于甲组(P<0.05),两组患者上下方向和前后方向摆位误差比较差异无统计学意义(P>0.05).乙组患者前后方向校准频率小于甲组(P<0.05),两组患者左右方向和上下方向校准频率比较差异无统计学意义(P>0.05).结论 胸部肿瘤放疗时,患者双手置于体侧的方式引起的摆位误差较小.
