钻孔引流治疗慢性硬膜下血肿48例临床疗效观察

摘要

Objective To explore the clinical efficacy of trephination with drainage in the treatment of chronic subdural hematoma,and to promote clinical application of the procedure.Methods The clinical data on 48 patients who had undergone trephination for chronic subdural hematoma during the period of August 2006 to February 2012 were retrospectively analyzed.Results 46 of the 48 patients were cured,with an effectiveness rate of 95.8％ and a mortality rate of 0％.Subdural hematoma recurred in five patients,with a recurrence rate of 10.9％.Three patients developed complications,with a rate of complication of 6.5％.40 of the 48 ( 83.3％ ) patients were identified as class 0 + Ⅰ by Bender grading when discharged,as compared with 14.7％ ( 7/48 ) of the patients before the procedure,with a statistically significant difference ( P ＜ 0.05 ).Conclusions Trephination with drainage is an effective,safe,reliable procedure for chronic subdural hematoma.It is worth popularizing chronically.%目的 探讨钻孔引流治疗慢性硬膜下血肿的临床疗效,推广该术式的临床应用.方法 选择2006年8月至2012年2月于我院行钻孔引流术治疗慢性硬膜下血肿的48例患者的临床资料进行回顾性分析.结果 48例患者中46例治愈,有效率为95.8％,死亡率为0,复发5例,复发率为10.9％,出现并发症3例,并发症发生率为6.5％.出院时Bender分级:0+Ⅰ级病例为占83.3％ (40/48),与治疗前14.7％( 7/48)比较,差异有统计学意义(P＜0.05).结论 钻孔引流治疗慢性硬膜下血肿是一种疗效显著、安全可靠的手术方式.适合临床长期推广应用.
