改良冰冻血小板制备方法的评价

摘要

目的 回顾性分析2008-2011年东莞市中心血站冰冻血小板制备及复融的情况,评价改良的冰冻血小板制备方法对复融后血小板不可逆聚集的影响.方法 统计2010-2011年两年我站使用75%二甲基亚砜(DMSO)溶液作为冷冻保护剂制备冰冻血小板后的复融情况,并以此作为实验组,统计2008-2009年两年我站使用二甲基亚砜原液制备冰冻血小板后的复融情况,并以此作为对照组.实验组血小板袋内加入75%二甲基亚砜溶液后,置-50℃速冻机速冻1h,再转移到- 80℃低温冰箱保存,即改良法;对照组血小板袋内加入二甲基亚砜原液后直接在-80℃低温冰箱保存,即经典法.复融后,肉眼观察发现血小板出现不可逆聚集,判为不合格.结果 实验组冰冻血小板复融后的血小板不可逆聚集率为0.37%(5/1352),而对照组冰冻血小板复融后的血小板不可逆聚集率为7.94%(98/1234),两组之间的血小板不可逆聚集率的差异有极显著性(P<0.01),改良法制备的冰冻血小板在临床使用中未见不良反应的发生.结论 采用75%的二甲基亚砜溶液作为冷冻保护剂添加到血小板悬液内,-50℃速冻1h,再转移至-80℃低温冰箱保存的冰冻血小板制备方案血小板不可逆聚集率较低,是一个较好的制备冰冻血小板的方法,值得推广应用.%Objective To retrospectively analyze the situation about the frozen platelet preparation and melting in Dongguan city central blood station from 2008 to 2011,to evaluate the improved preparation method for the frozen platelet and the influence of irreversible aggregation after melting.Methods Statistical situation of the frozen platelet after melting since we used 75% DMSO as cryoprotectants to make the frozen platelet from 2010 to 2011 was carried out,and as the experimental group.Statistical situation of the frozen platelet after melting since we used DMSO stock solution as cryoprotectants to make the frozen platelet from 2008 to 2009 was also conducted,and as the control group.The experimental group was frozen for one hour in the -50℃ Instant Freezer after adding 75% DMSO solution,then transferred to -80 ℃ refrigerator,that was the improved method.The control group was frozen in -80 ℃ refrigerator after added DMSO stock solution,which was the classical method.If we visually found playlet irreversible aggregation after melting then the judgment was unqualified.Resula The rate of irreversible platelet aggregation was 0.37%(5/1352) in the experimental group after melting and there was no untoward effect in clineal applieation,the rate of irreversible platelet aggregation was 7.94%(98/1234).Between the two groups of irreversible platelet aggregation rate there was very significant difference (P < 0.01).Conclusions With 75% DMSO solution instead of DMSO stock solution as cryoprotectant added to the platelets suspension,-50℃ quickfrozen I hour and then transferred to -80 ℃ refrigerator,which is a very good method to make frozen platelet with very low rate of the irreversible platelet aggregation.We think it is worthy of wide popularizalion and application.
