皮肤轮匝肌瓣悬吊术对中、重度先天性上睑下垂患者睑缘活动度及美观性的改善效果

摘要

Objective To investigate the effect of skin orbicular muscle flap suspension in improving eyelid margin activity and aesthetics of patients with moderate or severe congenital ptosis.Methods 80 cases of moderate or severe congenital ptosis treated in our hospital during March 2014 and March 2015 were selected as the study objects,and randomly divided into control group and observation group with 40 cases in each group.Patients in observation group were given skin orbicular muscle flap suspension,while patients in control group were given frontal muscle suture suspension.Compared the surgical effect,eyelid margin activity and aesthetics,and post-operative complications in both groups.Results The total effective rate and satisfactory rate of aesthetics in observation group were 97.5％ and 70％ respectively,and 85％ and 47.5％ in control group,with statistically significant differences between two groups (P＜0.05).The eyelid margin activity in observation group was (4.81±1.43) mm,which was obviously higher than (4.20±1.04) mm in control group,with statistically significant difference between two groups (P＜0.05).The occurrence rate of post-operative complications in observation group was 7.5％,which was obviously lower than 25％ in control group,with statistically significant difference between two groups (P＜0.05).Conclusion Skin orbicular muscle flap suspension is proved to be effective in patients with moderate or severe congenital ptosis,which can significantly improve the eyelid margin activity and aesthetics.%目的 研究皮肤轮匝肌瓣悬吊术对中、重度先天性上睑下垂患者睑缘活动度及美观性的改善效果.方法 选择2014年3月至2015年3月我院80例中重度先天性上睑下垂患者作为研究对象,采用随机数表法分为两组,每组40例.观察组行皮肤轮匝肌瓣悬吊术,对照组行额肌缝线悬吊术.比较两种手术疗效、患者眼睑活动度美观性改善效果及术后并发症情况.结果 观察组治疗总有效率和美观性满意率分别为97.5％和70％,对照组分别为85％和47.5％,组间比较差异有统计学意义(P＜0.05).观察组眼睑活动度为(4.81±1.43)mm,显著高于对照组的(4.20±1.04)mm,差异有统计学意义(P＜0.05).观察组术后并发症发生率为7.5％,显著低于对照组的25％,差异有统计学意义(P＜0.05).结论 皮肤轮匝肌瓣悬吊术治疗中重度先天性上睑下垂患者疗效显著,能显著提高患者眼睑活动度,改善美观性.
