注意力转移对择期甲状腺腔镜手术患者术后疼痛不适及焦虑的影响

摘要

Objective To discuss the effect of attention diversion in influencing post-operative pains and anxiety of elective thyroid endoscopic operation patients.Methods 110 elective thyroid endoscopic operation patients treated in our hospital during April 2013 and April 2015 were selected as the study objects and randomly divided into control group and observation group with 55 cases in each group.Patients in control group were given regular care,while patients in observation group were given attention diversion combined with regular care.The pain degree was evaluated by visual analogue scale (VAS) before operation and at 2 days after operation,and the anxiety level was assessed with Hamilton anxiety scale (HAMA).Recorded hospital stays of both groups.Results The ratio of 0-1evel patients in observation group was 52.73％,which was obviously higher than 23.64％ in control group,with statistically significant difference (P＜0.05).Post-operative VAS and HAMA score in observation group were obviously lower than those in control group [(1.18±1.08) vs.(2.79±1.22),(8.64±2.69) vs.(10.02±3.84)],with statistically significant differences (P＜0.05).The hospital stays of observation group was (4.59±1.18) d,which was obviously shorter than (5.84± 1.27)d of control group,with statistically significant difference (P＜0.05).Conclusion Attention diversion can relieve the pain degree of elective thyroid endoscopic operation patients and improve the anxiety.%目的 探讨注意力转移对择期腔镜手术患者术后疼痛不适及焦虑的影响.方法 病例选取2013年4月至2015年4月收集的择期甲状腺腔镜手术患者110例,采用随机对照法将患者分为两组,每组55例,对照组给予常规护理,观察组在对照组基础上进行注意力转移干预,评估两组术后疼痛程度.评估患者术后疼痛程度,术前及术后2d采用视觉模拟评分(VAS)评估疼痛程度,采用汉密尔顿焦虑量表(HAMA)评分评估焦虑程度,记录两组住院时间.结果 术后0级观察组(52.73％)显著高于对照组(23.64％),差异有统计学意义(P＜0.05);观察组治疗后VAS评分和HAMA评分显著低于对照组[(1.18±1.08)vs.(2.79±1.22)、(8.64±2.69) vs.(10.02±3.84)],差异有统计学意义(P＜0.05);观察组住院时间显著短于对照组[(4.59±1.18) dvs.(5.84±1.27)d],差异有统计学意义(P＜0.05).结论 注意力转移能够减轻甲状腺腔镜手术患者术后疼痛疼痛程度,改善焦虑情绪.
