血液灌流联合连续性静脉-静脉血液滤过在急性有机磷农药中毒救治中的应用观察

摘要

目的 观察血液灌流(HP)联合连续性静脉-静脉血液滤过(CVVH)在急性有机磷农药中毒(AOPP)救治中的应用效果.方法 选择2015年5月至2017年5月本院收治的AOPP患者82例,将其随机分为对照组和研究组,每组41例.对照组患者采取HP治疗,研究组患者采取HP联合CVVH治疗.比较两组患者生存率、死亡患者生存时间以及治疗前后全血胆碱酯酶活力、炎性因子水平.结果 两组比较患者生存率、治疗前后全血胆碱酯酶活力以及治疗前TNF-α、IL-6水平相比较,差异无统计学意义(P＞0.05);研究组死亡患者生存时间长于对照组,治疗后TNF-α、IL-6水平低于对照组,差异有统计学意义(P＜0.05).结论 HP联合CVVH治疗AOPP可有效延长患者生存时间,降低中毒后机体炎症反应程度.%Objective To observe the effect of hemoperfusion (HP) combined with continuous venovenous hemofiltration (CVVH) in the treatment of acute organophosphorus pesticide poisoning (AOPP).Methods Eighty-two patients with AOPP admitted to our hospital from May 2015 to May 2017 were selected and were randomly divided into control group and study group,41 cases in each group.The patients in the control group were treated with HP,and the patients in the study group were treated with HP+CVVH.The survival rate and the survival time of the died patients of the two groups were compared,as well as the levels of whole blood cholinesterase and inflammatory factors before and after treatment.Results There were no statistically significant differences in the survival rate,whole blood cholinesterase before and after treatment,and the levels of TNF-α and IL-6 before treatment between the two groups (P＞0.05).The survival time of the died patients of the study group was longer than that of the control group,the levels of TNF-α and IL-6 of the study group after treatment were lower than those of the control group,with statistically significant differences (P＜0.05).Conclusion HP combined with CVVH can effectively prolong the survival time of patients and reduce the degree of inflammatory response after poisoning.
