脑脊液胱抑素C对视神经炎的诊断价值

摘要

目的 探讨脑脊液中胱抑素C(CysC)的水平与视神经炎的关系.方法 选择视神经炎患者46例;对照组患者86例,包括26例颅内高压,21例眼肌麻痹,以及39例其他神经系统疾病的患者.采用免疫比浊法测定脑脊液中CysC的水平,同时检测两组患者脑脊液中髓鞘碱性蛋白酶(MBP)水平.比较脑脊液合成率和脑脊液生化指标的差异.结果 视神经炎患者脑脊液中CysC水平明显低于对照组,差异有统计学意义(P<0.05),MBP和脑脊液合成率在两组患者中差异无统计学意义(P>0.05).受试者工作特征曲线分析显示,CysC对视神经炎诊断的曲线下面积为0.640.结论 脑脊液中CysC是反映神经系统炎症状态的一个指标,测定脑脊液中CysC水平有助于视神经炎的诊断和鉴别,并可能对疾病进行疗效评估和预后判断.%Objective To evaluate the level of cystatin C (CysC) in cerebrospinal fluid of patients with optic neuritis and other neurological diseases .Methods Totally 46 patients with optic neuritis were selected ,and 86 patients in the control group ,including 26 cases of intracranial hypertension ,21 cases of ophthalmoplegia ,and 39 cases of other neurological diseases .The level of CysC in cerebrospinal fluid was measured by immunoturbi-dimetry ,and the myelin alkaline protease (MBP) level in cerebrospinal fluid of two groups was detected .The difference between cerebrospinal fluid synthesis rate and cerebrospinal fluid biochemical indexes was com-pared .Results The level of CysC in the cerebrospinal fluid of the patients with optic neuritis was significantly lower than that of the control group ,the difference was statistically significant (P<0 .05) .There was no sig-nificant difference in MBP and the ratio of cerebrospinal fluid synthesis in the two groups (P>0 .05) .The re-ceiver operating characteristic curve analysis showed that the area under the curve of CysC for the diagnosis of optic neuritis was 0 .640 .Conclusion CysC in cerebrospinal fluid is an indicator of the inflammatory state of the nervous system .The determination of CysC in cerebrospinal fluid is helpful for the diagnosis and identifi-cation of optic neuritis ,and it may be used to evaluate the curative effect and predict the prognosis of the dis-ease .
