人工股骨头置换术与全髋关节置换术治疗老年人股骨颈骨折的疗效比较

摘要

目的：对比分析人工股骨头置换术与全髋关节置换术治疗老年人股骨颈骨折的临床效果。方法选择中山市坦洲医院2013年11月至2014年12月收治的老年人股骨颈骨折患者74例，随机分成对照组和治疗组，每组37例。对照组采用人工股骨头置换术，治疗组采用全髋关节置换术。观察两组患者治疗效果。结果两组患者手术时间、术中出血量、并发症、优良率比较，差异均有统计学意义（P ＜0．05）；两组患者术后引流量和总有效率比较，差异无统计学意义（P ＞0．05）。结论人工股骨头置换术与全髋关节置换术治疗老年人股骨颈骨折效果均较明显，但全髋关节置换术具有并发症少、关节功能恢复好等优势，值得推广应用。%Objective To make a comparative analysis of clinical efficacy of hemiarthroplasty and total hip arthroplasty in the treatment of elderly patients with femoral neck fracture.Methods 74 elderly patients with femoral neck fracture were selected by Tan Zhou Hospital of Zhongshan City from November of 2013 to December of 2014,which were randomly divided into the con-trol group and the treatment group,37 cases in each group.The control group was given hemiarthroplasty,the treatment group was given total hip arthroplasty.Clinical effects of the groups were observed.Results There were statistical diffrences in dura-tion of surgery,blood loss,complications and excellent and good rate between two groups (P <0.05).There was no significant difference in the postoperative drainage volume and total effective rate between two groups (P >0.05).Conclusion Both hemi-arthroplasty and total hip arthroplasty have good therapeutic effects in the treatment of elderly patients with femoral neck fracture, but Total hip arthroplasty has the advantages of less complications and good joint function recovery,which is worthy of populariza-tion and application.
