锁定钢板治疗肱骨外科颈骨折38例的临床观察

摘要

Objective To observe the effect of locking plate in the treatment of surgical neck fracture of humerus.Methods 76 patients with surgical neck fractures of the humerus from January, 2015 to February, 2017 were enrolled. The patients were dividedinto the control group and the observation group according to the random number table, with 38 cases in each group.The control groupwas treated with the proximal humerus anatomical plate, and the observation group was treated with the proximal humerus lockingplate. The treatment effect was compared between the two groups.Results The operation time of the observation group was (54.18±20.51) min, the intraoperative blood loss was (96.52±15.89) ml, the postoperative hospital stay was (8.14±3.15) d, and the frac-ture healing time was (96.28±23.87) d. The indexes were significantly lower than those of the control group (70.58±28.12) min, (120.51±19.58) ml, (10.56±3.86) d, and (115.36±34.00) d (P＜0.05). 6 months after operation, the mobility of the shoulderjoint in the observation group was significantly better than that in the control group (P＜0.05). The excellent rate of Neer's shoulderfunction score in the observation group was slightly higher than that in the control group, but the difference was not statistically signifi-cant (92.11％ vs 86.84％, P＞0.05). No complications, such as avascular necrosis, nonunion and malunion of the humeral head, were observed in the observation group.Conclusion Locking plate is effective in the treatment of surgical neck fracture of humerus, shoulder function recovery is good, and postoperative complications are few, safe and reliable.%目的 观察锁定钢板治疗肱骨外科颈骨折的疗效.方法 收集佳木斯中医院2015年1月—2017年2月收治的76例肱骨外科颈骨折患者, 按照随机数字表法分为对照组与观察组, 各38例.对照组给予肱骨近端解剖钢板治疗, 观察组给予肱骨近端锁定钢板治疗, 比较两组治疗效果.结果 观察组手术时间为 (54.18±20.51) min、术中出血量为 (96.52±15.89) ml、术后住院时间为 (8.14±3.15) d、骨折愈合时间为 (96.28±23.87) d, 均明显少于对照组的 (70.58±28.12) min、 (120.51±19.58) ml、 (10.56±3.86) d、 (115.36±34.00) d.术后6个月, 观察组肩关节活动度明显优于对照组 (P＜0.05) .观察组Neer肩关节功能评分优良率稍高于对照组, 但比较差异无统计学意义 (92.11％VS 86.84％, P＞0.05) .观察组术后未见肱骨头缺血坏死、骨不连以及畸形愈合等并发症出现.结论 锁定钢板治疗肱骨外科颈骨折疗效确切, 肩关节功能恢复良好, 术后并发症少, 安全可靠.
