认知干预对老年哮喘合并过敏性鼻炎患者认知及治疗情况的影响

摘要

目的：研究观察认知干预在老年哮喘合并过敏性鼻炎患者的治疗中的应用效果。方法回顾性分析2012－01～2013－12间在我院呼吸内科治疗的90例老年哮喘合并过敏性鼻炎患者，根据是否实施认知干预随机分为干预组与对照组，比较两组患者呼吸纠正时间、住院天数和治疗效果，患者肺活量（ VC）、1秒钟用力呼气容积（ FEV1）、FEV1／FVC等肺功能指标、外周血中lgE变化和哮喘急性发作时的病情严重程度之间的差异。结果干预组的患者呼吸纠正时间、住院时间少于对照组，而患者的满意度、治疗效果优于对照组，干预组患者的肺功能指标均比对照组患者的肺功能高，其差异有统计学意义（ P＜0．05）。外周血中lgE变化无明显差异。结论认知干预应用于哮喘合并过敏性鼻炎的治疗可以有效提高治疗效果，减轻病情严重程度，减少住院时间，并且有利于医患关系的改善。%Objective To study the application effect of cognitive intervention on senile asthma complicated with allergic rhinitis pa-tients.Methods A retrospective analysis of 90 cases with senile asthma complicated with allergic rhinitis in respiratory department of inter-nal medicine in the hospital from January, 2012 to December, 2013 was retrospectively analyzed.And they were divided into intervention group and control group.Correct aspiratory time, the days of hospitalization and treatment effects, vital capacity ( VC) , 1 second forced ex-piratory volume ( FEV1 ) , the difference between the acute episode of asthma FEV1/lgE change and FVC of pulmonary function in peripheral blood index, the degree of severity were compared.Results Patients with respiratory correct time and hospitalization time in the interven-tion group is shorter than that of control group.The patients′satisfaction and the treatment effect are better than the control group.The inde-xes of lung function in patients in intervention group are lower than the control group, with statistical significant difference ( P<0.05 ) . There was no significant difference in peripheral blood lgE changes.Conclusion The cognitive intervention should be used for the treatment of asthma complicated with allergic rhinitis, can improve the effect of treatment effectively, reduce the severity of the disease, reduce hospi-talization time, and help to improve doctor-patient relationship.
